0632078271 2016-03-31 Production Wells No Violations Land owner uses this well for house gas. There was a 1000 gallon tank with 750 gallons of oil in it on location. Land owner is Charles F Chancy 36220 Zion Rd. Rutland, Oh 45775. I instructed him I would have Columbus mail him out a packet he needs to fill out and return to become a domestic well owner in the State of Ohio. I further instructed him the next time he has the tank cleaned out to move it 50 away from the well.
1021771447 2016-03-30 Administrative Inspection No Violations I met Mr. Hall and Billy Putnam at the site of this well. Billy Putnam is going to plug the well for Mr. Hall because it is in the road of his gravel pit mine activities. I gave them the API #, the depth, well name and that it was a berea well. I explained that if the well was drilled with cable tools it could be plugged with clay. I told him I would give him Mr. Opritzas number in Columbus and a list of acceptable clay suppliers. I also explained there might be further questions with it being so close to a gravel pit.
