0355715167 2015-07-24 Orphan No Violations My inspection this day was needed to update the file and get a current look at the well. I was accompanied by Gene Chini, DOGRM Orphan Well Program Manager. We found the well still leaking brine water and bubbling gas up through the water. The location had dried up some but there was still a long scald area below the well and tanks. There seems to be a large cavity at the surface and gas is bubbling from the entire area, not just around the wellbore. It is believed this well was originally drilled to the Berea Ss, then plugged and sometime later cleaned out to produce gas for the house from a shallow zone.

1322632512 2017-04-26 Orphan No Violations On location with Orphan Well Program Manager Gene Chini to assist the DOGRM Radiological Safety Section with a soil sampling event at this orphan well location. A spud bar was used to probe into the fluid-filled depression around the 2.375 diameter tubing in an effort to locate a surface casing. No casing was found to be present. A plan will be developed to establish well-control and prevent further contamination, which will allow for contaminated soils to be cleaned up and disposed of prior to plugging the well.

1402187693 2014-05-06 Production Wells No Violations I inspected this well after speaking with Inspector Josh Garland about receiving a complaint that it was leaking brine water out of the well head and onto the ground leading to a drainage ditch. The well is listed as historic owner in RBDMS and is also listed as a dry hole that was plugged and abandoned in 1950. Upon inspection, this well consists of 2 tubing sticking approximately 4 above the ground surface. The surface casing is not visible due to the amount of corrosion and brine water present around the well head. I witnessed the well leaking large amounts (approx. 1 bbl over 15 minutes) of brine water with a strong aroma of hydrocarbons from the 2 tubing where the flow line had corroded off. There is a mechanical separator and drip near the well head, but they are no longer connected to the well. A 50 bbl plastic tank is also located near the well and was likely used for swabbing fluid off the well at one time. According to the property owner, Jim Young, there is always water at the base of the 2 tubing and it is always gurgling and bubbling from what appears to be gas escaping. This indicates that any associated casing in this well is badly corroded and has holes in it. I pulled a sample of the brine water as it was flowing out of the 2 tubing and sampled it for chlorides. The sample maxed out the high range Quantab Test Strip , which indicates a chloride level above 6000 parts per million. The brine water has created a kill zone approximately 10 wide by 60 long. This kill zone is covered in footprints from various wildlife rooting around in the salty soil. I have notified Inspectors Cynthia VanDyke and Josh Garland of the situation and will accompany them to the well site to evaluate it for the Idle and Orphan Program.
1469197943 2016-07-13 Orphan No Violations Upon inspection of this orphan well, it was observed that the well is still releasing brine periodically onto the surrounding soil. Vegetation kill areas are still present around the well location. The old 2100 gallon plastic tank has been removed from the location. The well was not releasing brine through the 2.375 tubing at the time of inspection, however brine and gas were bubbing to the surface in the annulus of the tubing. New photos were taken of the location.

Upon inspection, it is observed that the rods inside the tubing are likely too corroded to pull separately from the 2.38 diameter tubing. The rods and tubing will likely have to be pulled concurrently with each other during the plugging project.

1923707354 2014-05-07 Orphan No Violations Associated with complaint #4697. Visited well site with inspectors Cindy VanDyke and LJ Parkins. Well is equipped with 2 casing, and was equipped with a combination unit at one point in the past. There are 1 rods inside the 2 tubing. There is a depression around the well head roughly 2 x2 feet and full of water. A regular low volume of gas was seen coming up through the water. No lines or production equipment on well. 1 and 2 both equipped with corroded fitting and open to air. Landowner claims the well unloads about a Barrel of water 4-5 times a day. There is an empty 550 gallong plastic tank on site inside a dike. The dike is full of fluids. The dike wall has been cut and allowed to drain into the yard. There is an area of contamintion below the dike roughly 40 x 25 feet. There is also a trench dug from the well head out into the field.
