Gray clay and fine cuttings in returns. Good circulation established. 10:42 AM Two joints of 4.5 washover pipe downhole. The proposed route for ingress/egress was walked out and discussed. Mr. Jenkins was supplied with a Right of Entry Release Form and asked to complete and return it to the Division along with a copy of the current property deed/title insurance. Ms. Ridenour stated that the Meigs SWCD has applied twice to be considered for the Landowner Pass-Through Payment Plugging Agreement.

8:00 AM MH Rig Crew and Appalachian Well Surveys (AWS) crew on location. 8:20 AM Inspector Parkins on location 8:30 AM AWS crew rigging up 12 shots to make a collar buster to shoot off the 2.38 dia. tubing. The shots are designed for use in 1.5 dia. tubing. 8:50 AM AWS crew running shots down 2.38 dia. tubing with sinker bar in place to see if it will go past tight spot at 338. The tools sat down at 338. 9:10 AM AWS crew running shots back down 2.38 dia. tubing without sinker bar in place and the shots went past the tight spot at 338 and down to a collar at 494. 9:15 AM AWS crew shoots tubing at 494 and the tubing came free. 9:30 AM MH rig crew hooks mud pump up to 2.38 tubing and begins pumping. Good circulation established, indicating the tubing is free in the wellbore. 9:35 AM AWS crew off location. 10:00 AM MH rig crew continues circulating on well. Pressure hose failure on rig, shut down to replace hose. 10:20 AM MH rig crew back to circulating. Sand and fine cuttings only in returns. 11:20 AM MH rig crew mixes gel tracer with cement pump and observes returns for gel to determine if there is a hole in the 2.38 tubing. Returns showed gel at approximately 40 bbls, indicating there are no holes in the tubing. 1:15 PM MH rig crew mixes and pumps 130 sacks of Type I/II cement weighing 15.6 lbs/gal. Displaced with 0.5 bbls of freshwater to a depth of 130 2:15 PM Crew pulls 2.38 tubing out of hole. 2 holes discovered in joint #15 (2 from bottom), however these holes did not affect the setting of the cement plug from 494-130. 2:50 PM MH rig crew runs bailer to 100 inside 10.75 drive pipe to ensure cement is well below casing seat. 3:20 PM Inspector Parkins off Location 4:30 PM MH rig crew off location.

Rig crew tagged cement at a depth of 135. 8:15 AM Inspector Parkins on location 10:00 AM Rig crew attempting to pull 10.75 drive pipe, but it will not come free. Rig crew will rig up a drive clamp on their stem and attempt to drive the pipe further downhole and then attempt pulling again. 10:20 AM Rig crew was able to drive pipe further downhole and then pull the 10.75 drive pipe free successfully. 11:15 AM 10.75 diameter drive pipe successfully removed from wellbore. 11:35 AM Rig crew runs 87 of 8.63 diameter surface casing into the wellbore. Rig crew leaves casing hung in bowl slips at ground level. 11:45 AM 11:45-12:15 rig crew takes lunch break. 12:20 PM Rig crew runs 130 of 2.38 diameter tubing to just above the cement plug. Rig crew will cement the 8.63 diameter surface casing and set the surface plug simultaneously. 1:30 PM Rig crew mixes and pumps 104 sacks of Type I/II cement mixed at 15.6 lbs/gal. Heavy cement returns observed at surface both inside and outside the 8.63 diameter surface casing. The cement stayed at the surface after stopping pumps. 3:15 PM Inspector Parkins off location. MH Rig crew will wait on cement and begin demobilization activities tomorrow. 4:30 PM MH rig crew off location.

8:00 AM Rig crew on location, resume washover operations with 4.5 washover pipe and milling collar. 11:15 AM Inspector Parkins on location. Rig crew eating lunch. Mark Hines informed that the well kicked off and brought a moderate volume of gas and water to surface out of the diverter and into the pit. The gas also cut the casing seat of the 10.75 diamter drive pipe and began flowing up its annulus. Operations are now shut down for the day. MH Rig crew will attempt to drive the 10.75 diameter casing further downhole tomorrow in an attempt to re-establish a competent casing seat. 1:25 PM Rig crew off location 2:00 PM Inspector Parkins off location.

9:10 AM Inspector Parkins on location to check progress of contractor demobilization. Service rig, mud pump, cement pump, and tubing trailer removed from location. 9:25 AM Checked status of plugged well and observed natural gas bubbling through the soil approxiately 2 away from the well casing. No gas is coming from inside the 8.63 surface casing or immediately near the casing annulus. A vault and vent system will need to be installed on the well to contain and safely vent the stray gas. 9:40 AM Inspector Parkins off location.

1468516345 2016-07-13 Orphan No Violations Upon inspection, it was observed that this well is idle and is likely orphan. The well area is overgrown with brushy vegetation. The well consists of 2.375 production tubing rising 4 from the ground to a tee. One fitting of the tee has a 2 pipe nipple, elbow, and bull plug. The other fitting has a 2 pipe nipple to a 2 gate valve, elbow, and 2 flow line which extends 10 east from the well before it is buried. The valve is in the closed position. There is fresh water ponded around the tubing on the ground covering an area 3 in diameter and 6 deep. There is gas bubbling out of the freshwater at a slow to moderate rate. The PhD6 gas meter did not detect an ambient reading of CH4 or LEL. The well is located on the Meigs County Soil and Water Conservation Districts Conservation Area and is near a mowed path used for walking. Access to the well is off of Old Lima Road (County Road 3) and would use the existing driveway to the conservation area. From the driveway, access to the well is through a gate and around the shelter house and restrooms to the left through a haymeadow, across a small drainage ditch, along a cut path through trees, and north along the woodline to the well. Steel plates would be needed to bring equipment across the ditch. The current SEM score is 206. Photos were taken of the location.

A section of guardrail will need to be removed temporarily to allow equipment access beyond the parking lot.

Rig crew stops to allow Mark Hines to weld a collar onto the casing. 2:10 PM Rig crew drives 10.75 drive pipe to refusal by competent bedrock at a depth of 64. 2:15 PM Rig crew re-attaching diverter head and flow line. 2:30 PM Rig crew running 4.5 washover pipe with milling collar back into hole. 3:20 PM 4.5 washover pipe running into hole slowly. 7 joints currently downhole. 8:00 AM Inspector Parkins and MH rig crew on location preparing to install vault and vent system on well to contain and safely vent stray gas which is being produced near the plugged well. #57 stone, Vault casting, vent lines, and Quikrete on location. 9:55 AM Steve Jenkins and Jenny Ridenour of Meigs County SWCD on location to decide on an appropriate location for the gas vent line. 11:10 AM MH rig crew excavates around 8.63 diameter surface casing and cuts casing 30 below grade level. 11:30 AM MH rig crew lays #57 stone gravel base and places the concrete vault casting in place around the well. Rig crew then mixes 10 sacks of Quikrete and pours a floor in the vault casting around the well. 1:00 PM MH rig crew installs 2 diameter galvanized steel vent line in excavated trench which runs approximately 50 to the north of the well into the woodline. 1:30 PM MH rig crew begins backfilling trench and compacting soil in 6 lifts with a plate compactor. 2:40 PM Vent line cemented in place with a 12 diameter post form and 10 sacks of Quikrete. 2:45 PM MH rig crew dumping dry sacks of Quikrete around the outside of the vault casting before installing vault lid and finishing backfilling and complacting. 3:30 PM MH rig crew installs gasket material, vault casting lid, and manhole cover. Backfill and compaction will be finished before crew leaves today. 3:35 PM Inspector Parkins off location
