The company then filled this pipe to a depth of 4 using 8 yards ready mix cement to stabilize the conductor casing. I was present for this operation.

0541326060 2012-11-08 Plug / Plug Back No Violations I inspected this well and found that Mercer Well Service had arrived on site and was rigging up. The service rig crew removed the top portion of the christmas tree type wellhead and then installed a B.O.P. ( Blowout Preventer Stack ). The crew then began to run a string of 2.875 in. back into the well to be used as a working string. 110 sections of the 2.875 in. tubing was delivered earlier to the wellsite and each section was approx. 32 ft. in length. A total of 3500 ft. of 2.875 in. tubing was run into the well to determine the fluid level. The well was then swabbed and the fluid was directed into a nearby 500 bbl. frac tank. The string of 2.875 in. tubing was then tripped out of the hole again.

Total depth of tubing and rods will be 3,175 and a 1500 pound Blow Off Preventer head with a standard 2.00 T with valves and checks will be installed below the stuffing box. No reports of injuries, incidents or spills reported.

1354132599 2012-11-06 Plug / Plug Back No Violations I inspected this well location ahead of the plugging operations that are to commence on 11/09/12. I found a large christmas tree type wellhead in place and the electric powered artificial lift pump was gone from the wellhead. The only other equipment still on site was an off road type forklift. I noticed that the former cellar area that was below the rig superstructure was filled with pea gravel and aprrox. 4 in. of standing water could be seen on top of the pea gravel. Also the former rat hole was still in place and it was filled completely to the surface with water. I utilized a Quani Tab testing strip to test for chlorides in the water at the former cellar area. The low range Quani Tab strip maxed out on the scale. I then used a high range Quani Tab strip and its result stopped at 8.7 units mg/L Cl. According to the strips, 8.8 units would equal 6,562 ppm Cl. Next, I tested the water that accumulated in the former rat hole for chlorides. I utilized a low range Quani Tab test strip and it maxed out on the scale. I then used a high range Quani Tab strip and its result stopped at 8.8 units mg/L Cl. According to the strips, 8.8 units would equal 6,562 ppm of Cl.
1354208706 2012-11-09 Plug / Plug Back No Violations I arrived on site and found that Gray Wireline Service was rigging up their equipment in order to set the CIBP ( Cast Iron Bottom Plug ). The CIBP was set at 3220 ft. and Gray Wireline Service riggged down their equipment. Mercer Well Service then ran 3220 ft. of 2.875 in. tubing back into the well. The tubing string was touched down on top of the CIBP and then pulled up a few inches. Next, O-Tex Pumping Service rigged up their cementing equipment and filled the hole with 40 bbls. of brine water and pressured up to verify that the CIBP was holding in place. After a short time of pumping, representatives from Devon Energy, O- Tex Pumping Service and myself observed the pressure gauge on the wellhead holding a pressure of approx. 4500 psi. After waiting ten minutes, the pressure was again verified to be holding at approx. 4500 psi. The pressure was then released on the well. O- Tex then pumped 3 bbls. of fresh water and tested their cement pumping lines to 1000 psi. and then circulated hole volume until the brine water was removed. For Plug #1, O-Tex pumped 25 sks. of Class H cement followed by 17.36 bbls. of fresh water for displacement of the plug. Plug #1 was placed from 3220 ft. to 3020 ft. The service rig crew then pulled 11 joints of the 2.875 in. tubing in order to set the next plug. For Plug #2, O-Tex pumped 31 sks. of Class H cement followed by 14.84 bbls. of fresh water for displacement of the plug. Plug #2 was placed from 2840 ft. to 22590 ft. The service rig crew then pulled 40 joints of the 2.875 in. tubing in order to set the next plug. For Plug #3, O-Tex pumped 19 sks. of Class H cement followed by 8.3 bbls. of fresh water for displacement of the plug. Plug #3 was placed from 1600 ft. to 1450 ft. The service rig crew then pulled 27 joints of the 2.875 in. tubing in order to set the next plug. For Plug #4, O-Tex pumped 25 sks. of Class H cement followed by 2.98 bbls. of fresh water for displacement of the plug. Plug #4 was placed from 73
