0724154050 2014-06-17 Production Wells Violations Noted Inspected production well on 06/17/2014 with Brittany Hupp. The tank battery consists of 1-50 bbl steel tank, 1-1,600 gal plastic tank, drip, and 1-vertical separator within an earthen dike. The diked area is located within a wooden privacy fence. A small fluid leak was noted at the piping connecting the two tanks that will need to be addressed. The tank battery is used by multiple wells; identification is posted for all wells utilizing it, however no permit numbers are listed. The well is equipped with a pump jack with an electric motor. Well identification is posted at the wellhead as required. Research into the production history of the well showed that no production has been filed since 1993, therefore the well must be plugged.
1235191781 2012-09-06 Production Wells No Violations Well was inspected on this date. No violations found operationally. There are four wells on this location and numbering and name are being checked to clarify well identification.
1349895582 2012-10-09 Production Wells No Violations I inspected this well and found one small pump jack with electric motor on the wellhead. I could not determine the pressure on the tubing or casing due to no gauges being installed. No visible leakage was seen around the wellhead or piping. The identification sign on the well incorrectly identified it as the WCAC #3 when it should be identified as the Tru-Fit #2. The area around the pump jack and wellhead was well maintained. The tank battery contained one 50 bbl. steel oil storage tank, one 50 bbl. steel brine tank and one oil and gas seperator. The dike was in good shape, but there was an excessive amount of weed growth within the tank battery. I detected no visible leakage of the tanks or piping. The tank battery was enclosed with a wooden privacy fence and I saw no identification sign on the fence or tanks. It was determined that this tank battery also handles production for the Tru-Fit #1, the Tru-Fit #5 and the Tru-Fit #6A wells. I spoke with Jim Steger, the maintenance manager for the Washington Court Athletic Club where this well is located and advised him of the incorrect identification sign on the wellhead and missing sign at the tank battery. Jim told me he will have the proper signs made and installed within the next ten days. He also advised me that gas produced from this well is used to heat the athletic club buildings and no gas is being sold. I also contacted the well owner, Bob Workman, at Tru-Fit Corp. and advised him that no production reports have been filed with the ODNR since the 1990s. Mr. Workman told me he will get these reports filed as soon as possible to bring the records up to date.
1753974217 2015-03-17 Production Wells No Violations Inspected production well on 03/17/2015 to follow-up on a previously-issued compliance notice. The tank battery consists of 1-50 bbl steel tank, 1-1,600 gal plastic tank, and 1-vertical separator located within an earthen dike; the diked area is located within an unlocked wooden privacy fence. The tank battery is used by multiple wells (4); no well identification is posted at the tank battery. The well is equipped with a pump jack with an electric motor. Well identification is posted at the wellhead as required. Will contact Mr. Workman with Tru-Fit to determine his plan forward. No other problems were noted at the time of the inspection.
1802407070 2007-12-19 Production Wells No Violations Well is in production and equipped with pump jack, rods and tubing. It is used to supply gas to health club. Cloudy, 35 degrees.
