1443540894 2015-09-10 Production Wells No Violations Inspected production well on 09/10/2015 at the request of the landowner. While the dike needs built up in a small area on the backside, there were no violations noted at the time of the inspection.
1535463500 2018-08-21 Production Wells Violations Noted I Conducted a compliance inspection of this well at the request of Mr. John Yovanno, his address is 1280 Medina Line rd. His concerns are as follows (Well not producing, the power line running from n Medina Line rd. to the well are hanging low to the ground and well and tank battery overgrown with weeds)

My inspection found well shut in at well casing, no chart in the meter box, the Identification post is for Pin-Oak Energy Partners LLC not current owner and tank battery consists of a 100 Barrel steel oil storage tank, a 50 Barrel steel brine storage tank and two vertical separators all contained with an Earthen dike. Data review of the ODNR files found the well Change of owner was filed 05-03-2018 to A Berry Enterprises LLC P# 740-260-0421 then I checked records for Production filed for year 2017 which company reported 0 amounts in both Oil and Gas. Phone Call to A Berry Enterprises LLC to have company address the Identification violation and to address the power line issue.

1552131252 2017-02-22 Production Wells No Violations

Conducted a production well inspection on 02-22-2017 at the request of a concerned residence regarding poorly maintained tank battery. This tank battery consists of 1-100 bbl. steel tank (rusted no visible leaks), 1-50 bbl. steel tank, these tanks have visible rust with no visible leaks at time of inspection and 2-vertical separators located within an earthen dike. Well identification is clearly displayed at the tank battery. The well is equipped with a pump jack with an electric motor. No fencing around either the tank battery or the well. No other problems were noted at the time of the inspection.

1645146316 2015-05-04 Production Wells No Violations Inspected production well on 05/04/2015 at the request of the landowner, who had some concerns about the location. The back of the dike still needs built up, but new well identification has been posted. Lease road still needs graded and built up from the tank battery to the wellhead. Spoke to Jerry Reeves with DA Waldron. He indicated that the stuffing box rubber had been replaced since the last inspection, and that they intended to work on the lease road over the summer, when theyre out working in the area. No problems were noted at the time of the inspection. Will follow-up to ensure that issues have been addressed.
1662137442 2015-08-06 Production Wells No Violations Inspected production well on 08/06/2015 to follow-up on a request from the surface landowner. The tank battery consists of 1-100 bbl steel tank, 1-50 bbl steel tank, and 2-vertical separators located within an earthen dike. Well identification is clearly displayed at the tank battery. A small section of the dike behind the separators is beginning to erode down and will need to be built up. Jerry Reeves with DA Waldron was notified of this issue and will have it addressed promptly. The well is equipped with a pump jack with an electric motor. No other problems were noted at the time of the inspection.
1878142201 2015-05-07 Production Wells No Violations Inspected production well on 05/07/2015 at the request of the landowner. Confirmed again with Jerry Reeves (DA Waldron) that landowners issue regarding the condition of the lease road from the tank battery to the wellhead would be addressed this summer. No problems were noted at the time of the inspection.
1939039581 2018-08-27 Production Wells No Violations The purpose of this inspection was to follow up on a landowner request and associated compliance notice. I met with Jim Berry, the well owner and confirmed that issues brought up by the landowner, Mr. Yovanno were being addressed.

At the time of inspection, brush was being cut to gain access to and reattach the utility pole. Mr. Berry advised me that the power company had additional safety items that were required before they would restore power. He stated that the items were being addressed. Weeds around the well and tank battery have been mowed. The identification was posted, and this violation was resolved.

2044000740 2015-04-21 Production Wells No Violations Inspected well at request of landowner. The tank battery consists of 1-100 bbl steel tank, 1-50 bbl steel tank, and 2-vertical separators located within an earthen dike. The dike needs built up behind the separators. Well identification is posted at the tank battery, but it is peeling off and needs replaced. The well is equipped with a pump jack with an electric motor. A very small stuffing box leak was noted, which will need to be repaired. The lease road is badly rutted from the tank battery to the wellhead and needs to be graded and have stone laid down. Jerry Reeves, with DA Waldron, was notified of these issues and will have them addressed as weather permits. Well will be monitored to ensure that these issues are addressed.
