1204140468 2008-02-27 Production Wells No Violations I performed a random field inspection. Both the wellhead and the tank battery were GPSd for the first time. The ID sign was posted and the tank battery was in good shape. There were no violations at this time.
1479482813 2016-11-08 Production Wells Violations Noted I received a phone call at 3:27 PM from Terry Whitehair North East Emergency Response Coordinator about a oil release at this location. Scott Miller Contracting LLC( SMCLLC ) was on location. The area impacted was 50X80. Estimated 6-8 barrels of oil/ brine was released. They dug a shallow pit 20X 30 2 deep and lined it with a 40X100 pit lined and removed the contaminated soil and covered it over for the night, until the arrangements could be made to dispose of at an approved land fill. They also dug a pit on the location if any liquids would drain into and be collected the next morning by a water truck. The incident was reported to Scott Miller by Mark Tirpak on 11/7/2016. On 11/8/2016 Scott arrived on location not knowing how much oil was released that is when he notified Terry Whitehair of the incident and then Terry notified me. Upon my inspection of the well I located a 1.5 collar on the lubricator that is very pitted and rust covered this needs to be replaced before the well can be returned to production. All contaminated soil to be removed to an approved landfill with a copy of the manfest. Location to be restored and grass seed (rye) to establish vegetation. Under 1509: 9-8-2 DOGRM should have been notifyed with in 30 minutes of the findings of the release.
1482854934 2016-11-18 Production Wells No Violations All contaminated soil has been removed and disposed of at an approve land fill. The area that was impacted has been reclaimed and restored and seeded with rye grass. The well is equipped with 1.5 tubing. There is a domestic hook up at the wellhead. The tank battery is equipped with one 210 barrel tank and one 100 barrel tank and a horizontal separator. The 1.5 collar on the lubricator has been replaced. I notified Mike Cutter owner of Cutter Exploration that the well can be put back in production. Compliance # 1479482813 can be closed.
1482857636 2016-11-09 Production Wells No Violations I performed a follow up inspection on this location. The contaminated soil has been contained. Northeast Oil Field services is on location removing the oil from the siphoning dam, there was an estimated 2 barrels of oil removed. Scott Miller is getting trucking and land fill information and will start removing the contaminated soil to the land fill.
1482858748 2016-11-14 Production Wells No Violations I performed a follow up inspection. There is oil in the siphon dam but is contained . Scott Miller will be on location to start removing the contaminated soil to Kimble Landfill. There were various trucks on location to haul the material to the land fill Scott Miller will get the manifest and mail to Terry Whitehair North East Emergency Response Coordinator when job is completed.
