0141531780 2015-06-04 Production Wells No Violations I performed a follow up inspection on the above location. On 4/14/2015 I called Russell Miller from Vanguard Oil & Gas and instructed him that there was some oil and oil stained gravel inside the tank battery that needed to be removed and disposed of. Upon my follow up inspection the oil and stained gravel had been remove and replaced and the dike was also repaired. ID signs are posted. No noted violations at this time.
0973630458 2005-12-28 Production Wells Violations Noted The tank battery is shared with permit 1672 and consists of 1-210 bbl storage tank, 1-50 bbl salt water tank and a dike. There is no wellhead identifiation.
1341498407 2012-07-05 Production Wells No Violations On 7/4/2012 at 10:02 AM, I received a phone call from DNR Inspector Chris Grimm, he received a call from Russell Miller ( Summit Well Service / Vanguard Oil and Gas Service) about a tank getting struck by lighting. I called Russell on 7/4/1201 at 10:10 AM and they were in the process of vacuuming the oil and brine from inside the tank battery. No oil or brine left the dike area. I notified Eugene Chini ( North Region Supervisor ) and ( Deputy Chief ) Robert Worstall about the incident. On 7/5/2012, I inspected the location. This is multi well tank battery with 2 pumping units, #1 (3409921671) and #2 (3409921672). There is one 210 barrel tank, one 50 barrel tank and one horizontal separator. No oil or brine had left the dike area there was 61( 82 barrels ) in the 210 barrel tank at the time of the incident. The top was ripped open the bottom had a small split at the weld. The oil was thick which held most of it in the tank. Vanguard Gas was on location within an hour and emptied the tank. They are in the process of removing and steam cleaning the tank. They will excavate and properly dispose of all contaminated soil. I will inspect daily.
