0022167947 2011-11-07 Production Wells No Violations The stock tank, tank top, lines, and separator were all removed from location. Sweet Meadows Farm Drainage is on site removing all contaminated soil and gravel from dike area. They are using a trac-hoe to dig down to clean clay soil and stockpiling contaminated material at south end of dike area for removal. Material will be disposed of at Sweet Meadows facility remediation pit. No oil, brine or contaminated soil was found outside of the dike area.
0808366065 2009-07-06 Production Wells No Violations I GPSd both the wellhead and the tank battery for the first time. I continued my walk around the tank battery and the wellhead and found that there were no violations at this time.
1320761330 2011-11-05 Production Wells No Violations Received notification of tank explosion at approximately 08:00 on 11/05/2011 by phone call from Rick Huffman of Eric Petroleum (330-904-2567). Explosion affected one 210 barrel stock tank, one 70 barrel poly brine tank and one separator. The tank battery is fed by two wells, #21272 and #21273, which have been shut in. The fire had been extinguished by the time of my arrival. The top of the stock tank had been blown approximately 216 north of the tank battery.The brine tank was severely melted in the fire. The stock tank remained in place within the containment dike. Rick Huffman reported that approximately 25 barrels of crude oil and 4 barrels of salt water brine were released from the tanks. All of the released fluids were contained within the earthen dike. No evidence of oil or brine contamination outside of the diked area. Sweet Meadows Farm Drainage excavating crew are on site flushing the spilled fluids to south end of dike with fresh water and vacuuming crude oil and brine out of dike with water truck. They will return Monday, 11/07/2011, to remove all contaminated soil from inside of the dike. Visually inspected both well heads, no leaks, will remain shut in for now. No electrical storms in area, tank explosion attributed to static electricity.
