0174078034 2018-03-19 Production Wells No Violations I performed a status check on the above location the well is equipped with 4.5 X 2 swedge. There is no tank battery. There was a plugging permit issued for this well (3/8/2018). I spoke with Matt Postialwait will he will be scheduling a service rig to plug the well.
1064741907 2018-01-17 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0730. A review of RBDMS indicated that no production has been reported since 2011. Access to the well and the location was snow covered. No identification was present. The 8.625 well head was above grade. The 4.5 production casing was equipped with a production cross. The cross contained two closed valves on the sides and a plugged swage on top. While on location I called and spoke with Alliance Petroleum Corporation field superintendent Matt Postlethwaite. Mr. Postlethwaite was not sure of the status of the well but continued that he believed the company planned to transfer the well to the surface landowner. I directed Mr. Postlethwaite to post correct identification within ten (10) days. I communicated to Mr. Postlethwaite that I would issue a compliance notice to plug and abandon the well. The well is currently incapable of production and not legibly identified. Alliance Petroleum Corporation shall post legible identification within 10 days and submit a written plugging plan to Inspector Kenneth Frye at the Ohio Department of Natural Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685, within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Alliance Petroleum Corporation will be notified if the plugging plan is approved or denied.
1538585041 2018-10-03 Production Wells No Violations The purpose of my inspection was to follow-up on compliance notice # 1064741907. At the time of my inspection correct identification remained at the well head. Access to the well head remained stable and drivable. No leaks were observed. ODNR-DOGRM issued a permit to plug and abandon well #2-0730 on 03/08/2018. While on location I spoke with Alliance field superintendent Rusty Morrison. Mr. Morrison stated he did not know when the well may be scheduled to plug.
1544448161 2018-12-06 Plug / Plug Back No Violations The purpose of my inspection was to follow-up on compliance notice #1064741907. At the time of my inspection plug and abandonment activities had been completed. Pits were filled and the location was graded level. No equipment was left on location. The operator will work with Inspector John Fleming to complete final restoration work. No further action is necessary at this time.
