0065811131 2016-09-08 Final Restoration No Violations I was on location the do the final restoration of this location. The well site has not been reclaimed. I called Tim McConaughey Supervisor for Enervest and asked when they were gone to finish. Tim thought that it was completed. He stated that it would be completed in two weeks.
0117173021 2016-06-27 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. This well is being plugged to satisfy the requirements of Compliance Notice #-1255913922 issued by Mahoning County Inspector John Fleming. Diamond tagged the top of the bottom plug (set on 6/24/16) at a depth of 4849 with the ripping tool and ripped the 4.5 casing at a depth of 3457. They recovered 110 joints of 10.5lb/ft. production casing that was visibly corroded and had nickel size holes through some of the collars. They will run tubing back in the hole tomorrow and set the remaining plugs on 6/29/16.
0204309935 2016-01-06 Production Wells No Violations I performed a status check on the above location. The tank battery is shared with well #2- 3409920854. There is two 100 barrel tanks and two horizontal separators. The well is equipped with 1.5 tubing. ID signs are posted at the well head. The ID sign is not legible on the tank. Upon my inspection of the well head I could hear gas coming up around the 8 5/8 casing. I call Larry Lower Field Forman for Entervest and informed him that something needs to be done to correct this problem. He said that he would let his Supervisor Doug Beighle know.
1098690797 2016-06-24 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. This well is being plugged to satisfy the requirements of Compliance Notice #-1255913922 issued by Mahoning County Inspector John Fleming. Diamond spotted 2.375 tubing at a depth of 5172. They landed the tubing in fill-up on 6/23/16 and this was the deepest they could run. Petroset rigged up to the tubing, pumped 14 barrels of water, and the tubing pressured up to 1000psi. They were unable to clear the tubing and it was decided to pull one (1) joint and attempt to establish circulation. A 30 joint was pulled to 5142 and was pulling tight the first 10-15 then appeared to come out of the bottom hole sediment. Petroset rigged up, cleared the tubing, and broke circulation with 26 barrels of water. They then mixed and pumped 7.3 barrels (30 sacks) of cement slurry and displaced the slurry to 4840 with 19 barrels of water. The well was packed off and shut-in for the weekend.
1199361678 2008-01-02 Production Wells No Violations I performed a random field inspection. GPSd the tank battery and the wellhead for the first time. ID sign was posted. The tank battery was in good shape. I found no violations at this time.
1255913922 2016-01-27 Production Wells Violations Noted On 1/7/2016, I met with Larry Lower, Field Foreman for Enervest, and talked to him about the gas leaking around outside of the 8------
1459779582 2016-03-28 Production Wells No Violations On 3/28/2016, I performed a follow - up inspection on the above location. This well has a Compliance Notice ( -1255913922 ) that was issued on 1/27/2016. The corrective action was to have taken placeby 3/29/2016. At this time there has been no action taken to fix the leaking 4.5 casing and 8 5/8 surface casing. The well is a plunger lift equipped with 2 3/8 tubing. The controller of the well is located at the separator. At present the 4.5 casing and the 2 3/8 tubing is open to line pressure which is at 40 psi.There is also a domestic homeowner connected to the wellhead. There is one 100 barrel production tank and one horizontal separator. I have spoken with Larry Lower, Enervest Field Supervisor at different times about a time when to expect this to be fixed. He stated that it was on the list to be corrected.
1862800412 2016-06-23 Plug / Plug Back No Violations I was on location on 6/23/16 with Diamond Oilfield Services and Enervest personnel to discuss plugging operation for this Clinton Sandstone well. This well is being plugged to satisfy the requirements of Compliance Notice #-1255913922 issued by Mahoning County Inspector John Fleming. The well has been leaking gas outside of the 8.625 surface casing indicating that this string, the 4.5 production casing, or both strings, are damaged and require plugging to remediate the issue. Diamond is going to finish spotting 2.375 tubing in the hole and plugging and abandonment operations will commence tomorrow (6/24/216).
2050727400 2016-06-29 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. This well is being plugged to satisfy the requirements of Compliance Notice #-1255913922 issued by Mahoning County Inspector John Fleming. Diamond has spotted 2.375 tubing at a depth of 3446 and Petroset rigged up to the tubing to spot a plug across the Big Lime. They established circulation with 80 barrels of water and then mixed and pumped 22 barrels of cement slurry. The slurry was displaced to 3110 with 14 barrels of water. Diamond pulled tubing to a depth of 786 and Petroset rigged up to the tubing. They broke circulation with 7 barrels of water and then mixed and pumped 12 barrels of cement slurry. The slurry was displaced to 586 with two (2) barrels of water. Diamond pulled tubing to a depth of 227 and Petroset rigged up to the tubing. They established circulation and then mixed and pumped cement until good returns were achieved at the surface. The remainder of the tubing was pulled and the hole was topped off. Inspector Fleming will check the top plug tomorrow and close the compliance notice if warranted.
