0012682975 2015-05-04 Orphan No Violations Visited location on 05/04/2015 to witness continued plugging operations at the Wolfe/Klekotta orphan well. Tubing had been run in the hole to 766 (755 of tubing with 11 mud anchor). Gibson & Associates on-site. Waited on Formation Cementing. Rigged up Formation Cementing and S & H Water Hauling. Circulation was established almost immediately after pumping. Cement samples caught during cementing operations weighed out to ~15.0#. Good cement was seen at surface. Began pulling tubing - pulled 22 joints, washing down joints as needed. Pump on well - circulation established. Pulled out remaining 4 joints and the mud anchor. Wash-up and rig down Formation and S & H. Secure well and location.
0355556563 2014-09-16 Orphan No Violations This inspection was conducted with DOGMRM Inspector Allie Sherwood on 9/16/14 to confirm GPS coordinates and obtain additional information to complete the orphan case file (1989-09-126, region case # 96-015). The information gathered is as follows:

1. New GPS coordinates: 41.51287, -82.00771 2. This well is located at address 32531 Lake Road. The well is located south of Lake Road and west of Armour Drive and not west of Hermann Drive as originally indicated. A corrected sketch and measurements will be prepared for the case file.

0695776757 2015-06-18 Orphan No Violations Visited location on 06/18/2015 at the request of Gene Chini to follow-up on the final restoration of the well. The area of the yard under which the well was located has subsided somewhat more than the previous visit, and may require a small load of topsoil to build it back up. Took pictures and sent to Gene Chini.
0870521622 2015-04-30 Orphan No Violations Visited location on 04/30/2015 to witness continued plugging operations at the Wolfe/Klekotta orphan well. Rigged up Kenderas Construction. Excavated out tree roots and other obstructions around well. Opened up cellar to allow room to work on the wellhead. Rig up welder, Mark Hassell to weld extensions onto the 5 and 3 that can be brought up to a workable level and threaded into. Fill in extended-cellar with stone to create a flat surface to work on. Tighten wellhead and finish rigging up to prepare to run in the hole with tubing in the morning. Secure wellhead and shut down operations.
0912324135 2015-04-27 Orphan No Violations Visited location on 04/27/2015 to begin constructing well pad. A steel tank was on-site, and the tree that had been located immediately next to the well had been cut down by the landowner. Ron Gibson was onsite with Joe and Jake Gibson, and a backhoe. Dug up tree roots and had stone delivered to build the location.
1001576578 2015-05-05 Orphan No Violations Visited location on 05/05/2015 to witness continued plugging operations at the Wolfe/Klekotta orphan well. Gibson and Associates and Timco arrived. Wait on weather conditions to improve. Rig up Timco. Top off hole with water. Shot with 5 shots at 84 1/2 - pipe not free. Made a second attempt - pipe free. Prepare to pull 3 and rigged down Timco. Pull all 3 out of the well. Run in well with bailer. Bailed well dry. Waited on Alexis Concrete. Made a few additional runs with the bailer, retrieving approximately 2-3 gallons of fluid. Rigged up Alexis Concrete. Grouted well to surface with a funnel, then displaced it approximately 3 for ease of cutting off casing. Rigged down Alexis. Secured well and location, and shut down operations for the day.
1116667766 2015-06-08 Orphan No Violations Visited location on 06/08/2015 at the request of the orphan well department to check on the status of the final restoration of the location. The grass is growing back well, but the ground is still soft and there are small amounts of settling in the general proximity of the well. Relayed findings to orphan well department.
1164427370 2015-04-28 Orphan No Violations Visited location on 04/28/2015 to witness continued plugging operations at the Wolfe/Klekotta orphan well. Moved in and rigged up Gibson and Associates rig. Attempted to thread into the casing numerous times and with numerous thread types with no success. Discussed issue with Gene Chini and Norburt Lowder. Shut down for day.

1400735683 2015-05-27 Final Restoration No Violations Visted location on 05/27/2015 to check on the final restoration status of the well. Location has been re-seeded and covered with straw. Grass is beginning to grow back very well. Final restoration passes.
1433164460 2015-05-13 Orphan No Violations Visited location on 05/13/2015 to witness final restoration work on the Klekotta/Wolfe orphan well. Top of cement help over weekend at ~4-5 below ground level - no leaks noted. Wait on Kenderas Excavation to arrive on-site with mini-excavator. Rig up Kenderas. Excavate down around well. Cut off casing ~3 below ground level. Set on steel plate with ODNR and 60547 on it. Fill in excavated area with bricks, stone, and leftover cement that had been scraped out of the tank. Spread some of additional stone on landowners driveway and hauled the rest to Gibsons yard. Kenderas Excavation brought load of topsoil to location, which was spread over the well and used to fill-in any ruts left by the rig. Took additional topsoil to Gibsons yard. Wagner Trucking on-site to pick up tank. Road at end of driveway was already in bad shape, but was additionally torn up somewhat in the process of loading the tank. Move off Wagner Trucking and Kenderas Excavation. Gibson and Associates re-seeded, fertilized, and spread straw over affected areas of the yard. Will follow-up to check on grass re-growth.
1525172353 2015-05-06 Orphan No Violations Visited location on 05/06/2015 to check on status of plugging at the Wolfe/Klekotta orphan well. The top of cement remained at approximately 3 below ground level, which was where it was displaced to previously. There was no indication of any leakage. Rig down Gibson and Associates. Rig remains onsite due to bad starter. Waiting for repairs, then will move off location.
1580837940 2014-09-16 Orphan No Violations Inspected orphan well on 09/16/2014 with Norburt Lowder to update GPS coordinates and photos. The well is located within a concrete vault. The well is being vented through 1 plastic pipe attached to a tree, approximately 15 above ground level.
2105124843 2015-05-07 Orphan No Violations Visited location on 05/07/2015 to check on status of location reclamation. Starter was repaired Wednesday evening, and the rig moved off location. Plan to work on location Friday.
