This follow up inspection was conducted on 06-29-15 as a final restoration status check. This inspection found all equipment used during the plugging operations has been removed. The site has been restored and good vegetative growth has been established.

0386329687 2015-03-09 Orphan No Violations This follow up inspection was conducted on 03-09-15 for emergency contract compliance.

1. The utility pole was removed late last week and the Contractor worked on Saturday to get the area prepared for rig to move on today. 2. Contractor personnel on site - Keith, Shan, Mason and Gorge. 3. Trucks/trailers arrived with the track hoe, fencing and rig mats. 4. The Contractor excavated around well casing and found concrete. They removed approximately 20 of soil and concrete. They blew the well down, which took approximately 10 minutes, and cut off the old well head. They welded on 8 5/8 diameter casing so well head control can be installed and then backfilled the excavation. 5. A smaller water truck and operator arrived with 300 gallons of fresh water and started to slowly feed the water into the 1 diameter tubing. 6. The well will be shut in until the drill out operation is started tomorrow.

- Security started at 18:00 hrs. and ended at 8:00 hrs.

- Both the crew and I arrived on location at 10:30 hrs . - Trailer with rig mats and fencing was unloaded and mats placed so rig can be positioned over the well.

- Trailer with mud pump, power swivel and rig floor and the trailer with half tank arrived on location.

- Rig supervisor and back hoe operator arrived at 13:00 hrs.

- Rig was positioned over well. Mud pump, power swivel and rig floor off loaded and set up. Placed half tank on south side of rig. Chain link fencing set up and securred around site.

On site from 7:30 hrs. to 17:30 hrs.

- The Contractor started the relief hole by using a 7.875 tri-

- The contractor prepared to set a 40 section of 8.625 diameter casing in the well that the control head will be installed on.  The Contractor welded a collar with teeth cut into it on the bottom of this casing.

- - The Contractor then installed the control head and prepared to drill the relief hole.

On site: 1100 hrs. to 1530 hrs.

3. Another piece of 1 diameter tubing, about 10 long, was removed and the drill rate stopped at approximately 19. The

        rig supervisor is fabricating another bit and is also bringing up a 4 cable tool bit and jars from his shop.

0663849094 2015-03-18 Orphan No Violations This follow-up inspection was conducted on 03/18/2015 for emergency contract compliance.

1. Inspector Allie Sherwood covered plugging operations from 8:00 AM - 12:30 PM. 2. Inspector Sherwood notified me that at a depth of 15 the mill they were using was worn out. Moore will redress the mill with new carbide and start to mill again. 3. I arrived on location at 12:30 PM and found the contractor using a 3.625 flat mill. 4. At a depth of 16 the Contractor tripped the mill out and it had a 1 diameter concave wear area near the center of the mill. The contractor then fabricated a carbide tooth bit from 2 3/8 diameter tubing to over drill the 1 diameter tubing that is cemented in the 5 5/8 diameter casing.


1000459103 2015-03-16 Orphan No Violations This follow-up inspection was conducted on 03/16/2015 for emergency contract compliance.

1. Night Security starts at 18:00 hrs to 8:00 hrs. 2. I arrived on site at 8:20 hrs. and the rig crew were waiting on the rig supervisor. 3. At 9:00 hrs. the rig supervisor arrived hauling a trailer containing the drill collars, two mills and one over shot bit. 4. The rig supervisor informed me that he thinks that the 1 diameter well tubing is off to the side of the 5 5/8 casing.

6. Milled for 60 minutes and reached 15.5. Tripped out the mill which showed wear around outside and slight wear on the carbide coating. 7. The contractor installed a 4.875 cross blade flat mill, which has carbide coated teeth in the shape of a cross. At 11:00 hrs. they tripped in the hole. After after 60 minutes of milling and still at 15.5, they tripped out of the hole and found that all the carbide was worn off the mill. 8. The decision was made to bail the well bore dry and run a downhole camera. 9. Phone call made to Orphan Well Program Manager Gene Chini to get the downhole camera from our New Philadelphia office to site in the morning.

1195547763 2015-03-20 Orphan No Violations This follow-up inspection was conducted on 03/20/2015 for emergency contract compliance.

1. After retrieving a piece of the 1 tubing yesterday, the contractor reconfigured the teeth on the 2.875 over drill bit to allow for better fluid and cutting circulation around the tubing.

2. Over drilling at 19 with another contractor fabricated carbide tooth bit. The drill rate is slow because of the hardness of the concrete. Made only 1 which makes our total drill out depth at 20.

3. After discussing the situation with the contractor and Orphan Well Program Manager Gene Chini, the decision was made to drill a relief well approximately 24 to 36 to the northwest of the Adamcek well. This relief well will be drilled to a depth that is below the bottom of the 5.625 casing in the Adamcek well. An attempt will be made to allow the relief hole to drift into the Adamcek borehole, below the casing. This will allow us to properly plug Adamcek well.

1212955021 2015-03-18 Orphan No Violations Arrived at location on 03/18/2015 to witness plugging operations at the request of Norburt Lowder. Moore Well Service was on-site. Ran in hole to continue drilling out old plug. Drilled on old plug for approximately 1 hour and made about 2.5 of progress. Tripped out to check on bit. The bit was severely worn - about 1/4 was lost off the bit diameter and up to 3/8 was lost in several spots. Contacted Norburt Lowder to determine plan forward. Decision made to re-dress the bit and go back to drilling. Keith Moore welded on the bit for 30 minutes to re-dress it. Waited for bit to cool down, then started back in the hole. Drilled for approximately 45 minutes, making about 1 of progress, then the bit stopped drilling. Tripped out to remove collar and run back in the hole with the first bit used to attempt to wash over the pipe. Started back in the hole to continue drilling plug. Norburt Lowder on-site to take over supervision.

This follow-up inspection was conducted on 03/25/2015 for emergency contract compliance.

Status of the relief well:

1. The 8.625 casing set at 33 and well control head was installed. 2. The flow lines were plumbed from the well control head to the pits. 3. Contractor rigged up 6.25 tri cone button bit on 2.875 diameter drill collars. 4. Started to drill at 10:40 hrs at a rate of 4 minutes per foot. At 11:08 (Depth 50) the relief well had a show of gas and started to take water (approximately 40 bbl.). 5. At 60 the drill samples showed bits blue shale, fire clay and bits metal . Well still taking water (20 bbl.). 6. Had to shut down due to pit liner failure. Will have to bring new one in morning.

- The natural gas being produced by this well has been reduced due to the well being loaded with

  freshwater, which has caused the shale, believed to be producing the gas, to swell.

- The contractor has repositioned the rig to drill a relief well approximately 18 to the southwest of

  the existing well.

- A second steel pit was set in the street to give additional fluid storage.

This follow-up inspection was conducted on 03/27/2015 for emergency contract compliance.

Relief well status:

   9:15 hrs - Started drilling at 133 at 4 minutesper foot.
   11:15 hrs - at 165 tripped out drill collars and changed from the  6.25 bit to a 3 7/8 bit.

I left location at 13:30 hrs. I received a call from Keith Moore at 14:30 hrs advising me that at

   approximately 197 the drilling samples started to show concrete he decided to shut down until  
   Monday. 

On site from 7:00 hrs to 13:30 hrs.

1430241686 2015-03-13 Orphan No Violations This follow-up inspection was conducted on 03/13/2015 for emergency contract compliance.

1. Security starts at 6 PM to 8:00 AM. 2. Contractor personnel on site: Keith,George and Mason. 3. Because of the lack of weight on the power swivel, the contractor rigged pull down cables onto the power swivel to increase the downward pressure during drilling. Using a 3.875 tri cone button bit to clean out the 5.625 casing. 4. Drilling was east to a depth of 10 then drilling slowed and at 15 the drill stem started to torque up. 5. Tripped out the 3.875 toothed bit and no wear was noted all the teeth of the bit and changed bit to a 4 collar with teeth cut into it , tripped back in to the 15 drilled slow then stated to act the same tripped out all the teeth were worn off of the bit. 6. Contractor plans to bring up different types and sizes of flat mills and heave drill collars on Monday.

1430751026 2014-10-30 Orphan No Violations 08:30 hrs to 14:30 hrs

This follow up inspection was conducted on 10-30-14 for emergency contract compliance. Contractor, Gibson and Associates, hand excavated around the PVC vent pipe at 901 7th Court to expose well head. The well was actively venting gas at the time. Gibson exposed the top of the well head at a depth of approximately 2. Further excavating found the well contained in a 4 diameter brick and mortar vault. The well was equipped with a C-clamp well head on 5.625 casing. Gibson installed a gauge on the well head which rose to a maximum of 30 PSI. Gibson then blew the well down, which took approximately 15 minutes. He then removed the C-clamp and the well cap and found 1 diameter tubing cemented in the 5.625 diameter casing. I used my 350 depth meter to check for obstructions in the 1 tubing. The weighted line traveled freely to 350. Upon removing the line from the well the bottom of the 1 tubing was located at approximately 250. The Contractor Ron Gibson and Associates, reinstalled the C-clamp well head, reinstalled the PVC vent pipe and filled in the excavation.

1430836887 2014-12-11 Orphan No Violations This follow up inspection was conducted on 12-11-14 for emergency contract compliance.

A meeting was conducted with several Columbus DOGRM staff members, to show and explain the perpetration at the site so the plugging operation can proceed safely.


1430917629 2015-03-17 Orphan No Violations This follow-up inspection was conducted on 03/17/2015 for emergency contract compliance. On-site with Orphan Well Program Manager, Gene Chini.

- Moore personnel on-site: Keith, Mason, George. - Intended to run the down hole camera but unable to bail the hole dry. - Used 1 diameter tubing to obtain a sample of the material the bottom of the hole at 15. It appears to be a mixture of concrete and metal cuttings. - Discussed the options of overshot drilling the 5 5/8 diameter casing, order a new mill from Baker Tool or resize the 3 7/8 mill to 3 5/8 and re-carbide the mill surface. - It was decided that Moore would drill a relief well, approximately 36 to the northwest of the shale well. This relief well will intercept the Adamcek borehole just below the 5.625 well casing establish communication between the two holes and allow us to conduct plugging operations on the Adamcek well.

1462063044 2015-04-01 Orphan No Violations

This follow-up inspection was conducted on 04/01/2015 for emergency contract compliance.

1. On site at 08:30 hrs and found the contractor circulating on the relief hole, which is at a depth of approximately 270. Water was noted circulating out of the 1 diameter tubing in the Adamcek well. This water was clear at first and after approximately 4 minutes it turned cloudy, indicating that the relief well is below the 5.865 casing in the Adamcek well.

2. Had a conference call with Orphan Well Program Manager Gene Chini and Deputy Chief Dave Claus regarding the current status of the relief well. The items discussed were:

3. That it appeared that we were to not able to totally intercept the Adamcek well bore with the relief well but we were able to get close enough establish communication between the two holes.

4. No gas was detected from either the relief hole or the Adamcek well.

5. The Contractor stated that he had his pump pressure up to 600 PSI when drilling the last joint of the relief well and the cuttings diminished indicating that they could be circulating into the Adamcek well bore.

6. Plugging operations will consist of squeeze cementing down through the relief well and into the Adamcek well bore through the area of communication between the two holes at 270 and circulate the cement up through the 1 tubing in Adamcek well.

1623274415 2015-03-30 Orphan No Violations This follow-up inspection was conducted on 03/30/2015 for emergency contract compliance.

- The crew tripped pipe into the well with the 3.875 diameter bit to start drilling at

             approximately 197.
        - Drilling rate is 2 to 3 minutes per foot. Drill cuttings consist of pieces of straw and wood      
              fragments.

- At approximately 229 the drilling became erratic. At 265 the crew tripped pipe and bit out of

              hole. They will bring another bit tomorrow.

1661577492 2015-03-23 Plug / Plug Back No Violations I visited the location of this well which is undergoing drill-drill out and re-plugging under Orphan Well program contract. At the time of my inspection visit, the plugging contractor (Moore Well Service) is in the process of drilling an offset borehole to bypass a previously installed cement plug. The area surrounding the well location consists primarily of single-family residences. ODNR Orphan Well Inspector Norbert Lowder is providing principal regulatory oversight during this operation.

Contractor Moore Well Service hired Edwards Tree Service to remove all trees and Low brush under the utility lines and around the well area.

1822766134 2015-03-31 Orphan No Violations Arrived on location on 03/31/2015 to witness plugging operations. Moore Well Services was on-site, running a single-shot survey tool down the new hole. The tool was already downhole (at ~150) upon arrival. Tripped survey tool out - single shot tool misfired. Reset single-shot survey and ran it back downhole. Tripped survey tool out; survey showed inclination of 1 degree. Ran tool back downhole to bottom (~250). Tripped survey tool out; survey showed inclination of 1.5 degrees. Began circulating the well. Bits of brick, wood, shale, etc. were circulated out of the hole. While circulating, communication was established between the original well. Continued pumping to strengthen communication between the two wells. While circulating, the formation drank approximately 25 bbl of freshwater.
1837426619 2015-03-12 Orphan No Violations
 This follow up inspection was conducted on 03-12-15 for emergency contract compliance. 

- Security starts at 18:00 hrs and ends at 8:00 hrs

- Field toilet on site.

- Orphan Well Manager and Administrator on site to check progress.

- Using fresh water from City of Lorain (color of water is reddish)

- Rig crew working on getting well head control installed. They had trouble getting head to slide on drill pipe

- Started to drill out the cement packer and 1 diameter steel tubing. Drilling time - 30 miniutes per foot.

- Pulled bit which is worn on one side, possibly due to the 1 tubing being skewed off to one side in the 5 5/8 diameter casing. Drilled an additional 3 and pulled equipment from the well.

- Crew fabricated a tooth bit from a 2 3/8 diameter collar and used it to drill an additional 6. Tripped out of hole and found this bit worn.

On site from 7:00 hrs. to 17:30 hrs.

2097467960 2015-03-31 Orphan No Violations This follow-up inspection was conducted on 03/31/2015 for emergency contract compliance. DOGRM Inspector Allie Sherwood covered todays activities.

Relief well status:

- Rig crew ran single-shot down hole deviation survey. The first survey timer misfired. The survey at 150 indicated 1 degree of hole deviation. The survey at 250 indicated 1.5 degree of hole deviation. - We circulated on the relief hole and we established circulation through the 1 tubing in the Adamcek well. This indicates that there is communication between the two holes at depth. - The Contractor hooked up to the Adamcek tubing and reverse circulated through the relief hole to check flow. - The Contractor drilled the relief hole to approximately 280. It appeared that we were close to but did not intercept the Adamcek well bore.
