0091402621 2015-04-07 Orphan No Violations Arrived on location on 04/07/2015 to witness plugging operations. Gibson & Associates were on-site and rigged up. Formation Cementing arrived and rigged up. Waited on water haulers to arrive. S & H arrived and began filling tank. Lined half tank in preparation for plugging. Started pumping. Circulation was established after pumping approximately 1 bbl. Well circulated out oil. Continued circulation, then pumped gel with flake ahead of cement. No problems were encountered while setting the bottom plug.

The cellar was full of fluid, which covered the casing. I noted bubbles were visible approximately every 15 seconds in the fluid, in the vicinity of the well casing. Some oil had also accumulated in the cellar which was flushed out with fresh water and ito the half tank. I checked the well with my Sperian gas meter and obtained a field test reading of 1% LEL. The contractor bailed the fluid out of the cellar/casing and found that the surface plug had dropped approximately 43. The contractor ran a down-hole camera into the well to see were the gas is entering the well. At approximately 25 we saw fluid entering well bore and at @ 40 bubbling was noted in the center of the fluid column.

1. Contractor on site to run the 5.5 diameter surface casing to approximately 150. Made several attempts but could not

      get to 150.  Pulled one 20 joint and set casing at 130. 

2. Formation Cementing cemented the surface casing in place using 50 sacks of Class A cement, mixed at 15.6 ppg.

1. The well is still leaking. Bailed fluid out down to the top of the cement (44), and then ran a down hole camera. Observed bubbling in the fluid at the base of the well bore only. There was no gas seen entering through the walls of the well bore.

2. Consulted with Orphan Well Program Manager Gene Chini. He advised Gibson to drill out surface plug and then replug the well.

3. Gibson arranged for Timco to bring out their mud pump and power swivel when it is done at the site they are working on for Steve Hagen in Cuyahoga County.

1. Drilled to approximately 155. Circulated drill mud three times to clean out well bore. 2. Had trouble tripping out the collars and 7.75 bit due to a tight hole. Pulled the stem while rotating and was able to remove the collars and bit. 3. Will run the 5.5 diameter surface casing in the morning.

* Excavated around well casing and installed a 30 plastic pipe around well casing as a cellar.

  • Cut the 10.75 diameter casing down to weld on a collar. While cutting the 10.75 casing the 5 diameter casing fell down hole.
  • Welded a collar on to the 3 diameter casing in order to install the stripping head for the 1.90 tubing.

1268419811 2010-03-12 Orphan No Violations Met with Tim Carter, Project Engineer, with Donleys. He represents the Cleveland Clinic. 400 NE of new hospital foundation is a abandoned Clinton well. Last known owner was Dempsay in 1936. It is shut in on 3 casing and has valve attached. No leakage at this time. It has 10, 5 and 3 in well. Cement has been poured between annular spaces. The well record is attached to file. Accsess to the well will be thru main entrance and off of a new parking lot for the new hospital. I mailed an application to Tim Carter and explained the program to him and associates. Area is under construction at this time. No well owner found in search.
1294951920 2014-05-01 Orphan No Violations Visited location on 05/01/2014 to meet with architect Tim Lannon, who is working on the Cleveland Clinic expansion project. The well is located ~8 from the curb in the northwest corner of the parking lot. At one point the well likely stuck out of the ground, but as the area around the well was built up ~6 in order to construct the parking lot, all that can be seen of the well is a 3 diameter steel pipe which sticks out of the ground ~6 with a plug on top. An open ended, ~1.5 diameter steel pipe is sticking up ~8 from the well. Occasional whiffs of gas were noted at the time of the inspection. Mr. Lannon was provided with information regarding well ownership and plugging procedures (should the Cleveland Clinic choose to plug the well themselves) and information regarding the orphan well program (should they choose to wait and allow the orphan well program to plug it).
1361084168 2015-04-08 Orphan No Violations Arrived on location on 04/08/2015 to witness plugging operations. Gibson & Associates was rigged up and onsite, as was Timco. Tagged top of cement at 1,578. Shot pipe at 1,190. When the first joint of pipe was pulled out, a 3 piece of 5 was hung up on the collar. Removed it, and then pulled out the first joint (pup joint, ~8-10). Pulled out 7 joints of pipe. Ran in hole with wireline. Sat down at 1,104; bottom of 3 found at 1,046. Decision made to shut down for the day, come back tomorrow, trip out 3, then run in hole with tubing to set Big Lime/rip plug. Rig down Timco.
1409141816 2014-06-16 Orphan No Violations Met on site today, 06-16-14, to discuss status of an old Orphan well (case # 10-001) which is along the northern most edge of the Valet Parking lot for the Cleveland Clinic. In the original case file the photos were taken in the initial phases of parking area construction and show the well was equipped with 10, 5, 3 diameter casing and what appears to be 1.5 flowline and the complete casing and well head assemply was approximately 5 tall. As of todays inspection the casing and well head assembly has been cut off and only the 3 diameter casing and 1.5 tubing is visible at ground level. Todays meeting is to get the buried portion of well casing exposed for updated photos and to check for known buried utilities in the well area. Mr. Sizemore the foreman for the general contractor, brought in a mini-track hoe to expose the casing. They excavated to a depth of approximately 2 and found:
  - 1.5 OD tubing (gas escape)
  - 3.5 OD tubing
  - 5.5 OD casing
  - 10.5 OD casing

This well was not currently equipped with a well head. The annulus between the 3.5 to 5.5 diameter casings and the annulus between the 5.5 to 10.5 diameter casings appears to be filled with cement The 10.5 diameter casing showed signs of it being driven (by a cable tool rig).

The electric supply line for the nearby lamp poles in the parking lot is approximately 6 to the north of the well casing.

New GPS coordinates were obtained: 41.43933, -81.98337.

1409754974 2014-09-02 Orphan No Violations The well is currently being evaluated for plugging through the Orphan Well Program. The well is near the edge of the newly constructed parking area for the Cleveland Clinic Richard E. Jacobs Healthcare Facility. Currently there is a 3-inch capped fitting, nearly flush with grade, approximately 6 inches from the edge of the curb, in the grassy portion. There are no other surface appurtenances. There is no evidence of leakage of gas or fluids.
1430490037 2015-03-31 Orphan No Violations This follow up inspection was conducted on 12-01-14 for emergency contract compliance. Orphan Well Program Manager also on site.

* Contractor rigged up their swab rig and control head to run a set of swabing tools in the well to determine its total depth.

  • The contractor started to run the tools into the 3. At approximately 1200 a small amount (1 quart) of heavy black oil leaked out from the 3 tubing through a corroded area at the top of the 5 casing. The oil was cleaned up and total depth was found at 2344.
1430501570 2015-04-02 Orphan No Violations I was not onsite today but had a phone conversation with plugging contractor Ron Gibson regarding well and site preparation. Gibson had a meeting earlier today with the construction contractor working for the Cleveland Clinic and discussed the size of the area he will need for his plugging equipment. This construction contractor will prepare the site for Gibsons equipment.

Gibson also excavated around well head and prepared well casing for the welder.

1. Timco well service on site and rigged up to log well. 2. Ran a CCL / Bond logging tool and logged to approximately 1578 which was the top of the cement. 3. Logging crew rigged up 15 shots to cut casing at approximately 1500. Ran shot assembly into well bore but could only get to 1208 (the top of lime is at approximately 954). 5. Shot the casing at approximately 1194 which would put the bottom of the Big Lime plug approximately 240 below top of Big Lime. 6. Contractor rigged up to pull 3 tubing

1437150518 2015-06-29 Final Restoration No Violations
 Follow-up inspection was conducted on 06/29/2015 for emergency contract compliance.

1. The 10 diameter casing was cut off approximately 36 below grade and an identification plate was tack welded over it.

2. The hospital construction contractor will be installing the vault and vent system and completing the restoration of the area around the well.

Final Restoration is approved.

1526052190 2015-04-21 Orphan No Violations This follow-up inspection was conducted on 04/21/2015 for emergency contract compliance.

1. Timco Well Service on site to drill out surface plug. They used a 7.75 Tri-cone tooth bit on 3.5 diameter drill collars. They tripped into the well bore to 44 and started to drill at a rate of 4 minutes per foot. At a depth of 92, drill cuttings show fine grained cement.

* Contractor ran the rig sand-line in to find total depth - tagged at 1100. * Pulled 3 diameter casing up to 550, then ran 1.90 tubing to 1104 . * Formation Cementing Company set the rip/Big Lime plug from 1104-800 through the 1.90 diameter tubing using 90 sacks of Class A cement, mixed at 15.6 ppg. Tripped tubing out of hole. * Pulled rest of 3 tubing and tripped in 1.90 tubing to 400. * Formation Cementing set the top plug from 400 to surface through the 1.90 diameter tubing using 190 sacks of Class A cement, mixed at 15.6 ppg. * Tripped 1.90 tubing out of hole and shut well in gas tight. * Tallied 3 at 1194 on trailer which will be hauled away for salvage.

This follow up inspection was conducted on 04-07-15 for emergency contract compliance.

The contractor installed a tubing head and valves and then ran hoses to the tanks. They lined the circulating tank and hauled in freshwater because the fire hydrant they were going to use was blocked by the construction equipment being used to build the hospital. The contractor ran 1.90 diameter tubing with a mud anchor to approximately 2325. They pumped 20 sacks of Class A cement, mixed at 15.6 pounds per gallon, and displaced it with 4.7-bbls of freshwater to approximately 1830. The pulled tubing and shut down.
