0004526658 2009-01-05 Orphan No Violations I inspected this well at the request of personnel in the Central Office. I did find a well casing located in an agricultural field. The casing was 4 inch and was coming up about 12 inches above ground. There was no indication that the well was being used. The casing was open and the fluid was approximately 100 feet down from the surface.
1248180306 2009-06-30 Production Wells No Violations This well was found while cutting a lease road to new well being drilled by Knox Energy. The new well permit number is 34-089-2-6000-0000. The well head is approximately 102 feet from the new well being drilled. Bill Mitten called me to inform me that they found this well and that it was leaking a small amount of gas. They have drilled to a TD of 663 feet on the new well and cemented in the surface casing. The drilling rig is temporarily shut down until this old can be cleaned out and properly plugged. This old well was once thought to be located across the road at 40.24109, 82.48288. However, that well is an old water well according to a landowner. There was once a barn there that has now been removed.
1601068752 2009-07-02 Plug / Plug Back No Violations This old well is being cleaned out by Knox Energy. A service rig with a power swivel was brought in and used to clean well out to a TD of 1669 feet. After considerable effort they were unable to clean well out any deeper. A Lime plug was set from 1669 to 1369. A Berea plug was set from 691 to 391. The surface is being temporarily left unplugged so that it can be monitored while new well is being drilled and completed. After the new well is drilled and completed this surface plug will be set.
2099044156 2013-07-17 Final Restoration No Violations All production equiptment and debris has been removed. Vegetation has been restored. Restoration passes.
