0026140185 2017-04-05 Production Wells No Violations This inspection was conducted as a status check. I was accompanied by the landowner Mr. Craig Srba. Mr. Srba contacted me with some concerns regarding the status of the company and alleged that he has not received royalty payments for over a year. The well was equipped with a pumping unit and motor. Identification was observed at the well. This well produces into a shared tank battery with API#5952 and API#5876. Identification was observed at the tank battery. The tank battery consists of (1) 100 barrel tank, separator, drip, and meter inside an earthen dike. A second set of equipment was observed for the Srba #1. No violations. Photos were taken of the well. Time of inspection: 3:00 PM.
1679413552 2011-08-08 Preliminary Restoration No Violations An inspection revealed that the Preliminary Restoration meets ORC 1509.072. A GPS reading was obtained for the well and tank location. The well was equipped and showed signs of production. The tank battery location consisted of (1) 100bbl tank w/no knockout, (1) separator, (1) drip and (1) meter for SRBA #3 and SRBA #2. A second set of equipment inside the earthen dike for SRBA #1.
