0057878161 2013-10-23 Preliminary Restoration No Violations This inspection was conducted as a preliminary restoration status check. The well was equipped with a pumping unit and motor. It was not pumping during inspection. The area around the well has been restored. The tank battery consists of (1) 100 barrel tank and separator inside an earthen dike. Identification is posted at the tank battery. Restoration meets ORC 1509.072.
0451021990 2019-04-16 Production Wells No Violations I conducted a follow-up on the gas leak. My inspection revealed that the leak has not been corrected. I contacted Howard Harrop during the inspection. Mr. Harrop said he shut in a valve that is located on the 2 inch steel line before the leak. Mr. Harrop said that it did not correct the leak as the line was back feeding. Mr. Harrop said he was on his way to shut in other wells in the area on the sales line and will have to add a valve on the other side of the leak to prevent the gas from back feeding. 1555346839 2019-04-15 Production Wells No Violations I conducted an inspection on this well today in regards to a gas leak that was called in by the landowner. My inspection revealed that gas was flowing uncontrollably from a 2 inch steel elbow fitting after the separator and a chart recorder at the tank battery. I immediately contacted Mr. Tom Giusti who is currently the receiver for Hopewell Oil & Gas Development. Mr. Giusti contacted Howard Harrop who then contacted me. I advised Mr. Harrop of the gas leak and he said he would take care of it today. I told Mr. Harrop to let me know when he has the leak corrected. I did not observe any violations at the wellhead. I observed identification at the tank battery. The tank battery consists of (1) 100 barrel tank, separator, and chart recorder inside an earthen dike. I contacted Andrew Adgate with the Emergency Response section at 1:45 PM and advised him of the gas leak. Time of inspection: 11:55 AM. Weather: 40 degrees F, Overcast
