0073596789 | 2017-01-03 | Production Wells | No Violations | I met with Division Hydrologist Kyle Tharp and Emergency Operations Coordinator Josh Sheiring at the house on the property of the well. Mr. Tharp was investigating the issue of oil in the spring water. Mr. Tharp took samples of oil from the spring and from the tank battery of this well. There are two drilled water wells on the property that Mr. Tharp inspected. One of the water wells is no longer in use and is located behind the house. The other water well is used for drinking water and is located on the west side of the house. A water level meter was used in both wells to determine static water level. No oil was observed on the meter probe. |
0600847167 | 2006-12-07 | Drill / Deepen / Reopen | No Violations | Formation Cementing cemented the surface casing. The 8 inch went to a depth of 441 feet. 130 sacks of Class A cement were used along with 300 pounds of Gel, 400 pounds of Accelerator, and 150 pounds of L.C.M. cotton seed hulls. There were good cement returns to the surface. |
0896661451 | 2010-05-26 | Annular Disposal | No Violations | I conducted a mechanical integrity test on this well today. The test began at 1:15 pm with 290psi and concluded at 2:15pm with 288 psi on the annulus. There was less than one percent loss during the hour long test. Joe Jones completed the MIT work on the well. There were no violations noted this date. |
0938591719 | 2017-02-16 | Production Wells | No Violations | I was on location today to meet with Greg Mason from NGO Development. Today I will witness NGO doing an integrity test to the 8 inch casing and an Echo test to check the fluid level. NGO pressured the well up to 200 pounds pressure before conducting the test on the well. The start time for this test was at 12:00 pm at 200 pounds. When the test was finished and I left location at 1:30 pm the pressure on the well was at 200 pounds. I have requested a copy of the echo test to show the level of the fluid. The test at the well showed the fluid level well below the surface water in the area. |
0977944149 | 2016-12-30 | Production Wells | No Violations | This inspection was conducted as a status check. Emergency Operations coordinator Nate Lane was present for inspection. NGO Development agreed to dig up the flow line to check for leaks. No leaks were observed. |

1274895748 | 2010-05-26 | Preliminary Restoration | No Violations | There is an active pumping unit on the well. The location has been properly graded and vegetation has grown. The lease road is properly stoned. There is a separator and two 200 barrel tanks contained within an earthen dike. There is proper display of identification posted at the tank battery. Preliminary Restoration passed. |

1399414214 | 2017-01-12 | Production Wells | No Violations | This inspection was conducted as a status check. NGO Development agreed to pressure test the 8 inch surface casing. A compressor was used to put 200 pounds on the 8 inch casing. The fittings were leaking on the 8 inch casing and were tightened. The 8 inch ring/pack-off rubber was also leaking. NGO did not feel that they could get the ring any tighter that would make the leak stop. At 12:00 PM the test began with 195 pounds. The gauge showed 175 pounds at 3:00 PM. A chart recorder was also used on the test. The recorder was set for a 24 hour chart run. Greg Mason of NGO Development was present for test. |
1483967669 | 2017-01-05 | Production Wells | No Violations | An echo meter test was performed on this well today to determine static fluid level in the annulus between the 8 inch surface casing and the 4.5 inch production casing. Southwest Region Supervisor Dan Goins and Southwest District Supervisor Eric Conrad were present. Greg Mason and Dean Limbacher of NGO Development conducted the test on the well. The test revealed that the static fluid level did not appear to be close to the bottom of the 8 inch casing. Time of inspection: 10:00 AM. |
1495212503 | 2017-05-18 | Production Wells | No Violations | This inspection was conducted as status check. An investigation is still on going regarding the oil in the spring on the property. Those present for the inspection were Southwest Region Supervisor Dan Goins, Southwest Region District Supervisor Eric Conrad, Environmental Program Manager Kyle Tharp, Hydrologist Jake Glascock, Emergency Operations and Response Manager Scott King, and Emergency Response Regional Manager Trisha Cartmell. No violations during inspection. Mr. Brad Wells is the grandson of the homeowner and lives at this residence. Mr. Wells was not home during the inspection. The spring was not observed due to dogs in the fenced in area of the spring. Time of inspection: 11:30 AM. |
1543122588 | 2017-01-13 | Production Wells | No Violations | This inspection was conducted as a status check. The gauge on the 8 inch casing showed approximately 60 pounds at 12:00 PM. |
1556196420 | 2019-04-25 | Production Wells | No Violations | My inspection revealed that the well has just been serviced. When I arrived a service rig owned by Derrick Buxton was being pulled off the well. Mr. Buxton told me they pulled and steamed the tubing. The investigation into the contaminated spring has been completed. No more oil has been observed in the spring. Time of inspection: 8:25 AM. Weather: 51 degrees F, Rain |
1622322446 | 2017-01-04 | Production Wells | No Violations | This inspection was conducted as a status check. This well and the spring is still being investigated due to oil in the spring water. Mr. Greg Mason with NGO Development was contacted and asked if they would be willing to conduct a mechanical integrity test or some other method to determine if this well is causing oil to be in the spring water. Mr. Mason agreed to do so. |
1732599759 | 2017-01-13 | Production Wells | No Violations | This inspection was conducted as a status check. The gauge on the 8 inch casing showed approximately 75 pounds at 8:30 AM. |
2099421148 | 2016-12-27 | Production Wells | No Violations | This inspection was conducted as a status check. The well and tank battery were inspected due to a spring water contamination issue. The leaks or problems were observed at the well or tank battery. The company informed me that the well is currently only producing house gas and is not being pumped due to parafin in the well. No violations noted. The spring is located behind the house on the property of this well. The landowner does not use the spring water for drinking water but instead uses a drilled water well near the house. |
