1508861012 2017-10-19 Production Wells No Violations This inspection was conducted in order to refer the well to the Orphan Well Program. The inspection revealed that the well was equipped with a pumping unit and electric motor. The well and pumping unit was completely covered in brush/vegetation. Identification was not observed at the well. According to RBDMS production has never been reported on this well. The well owner Mr. Jack Swick is deceased. RBDMS research revealed Mr. Swick did not have a bond with the Division of Oil and Gas but instead sworn financial statement. The well produced into (2) 100 barrel tanks located at these coordinates: N 40.00496 W -82.35583. Identification was observed at this tank battery with the name J. Swick written on one of the tanks. A 100 barrel unloading tank was observed near Flint Ridge Rd. at these coordinates: N 40.00048 W -82.35686. An inspection of this tank revealed a leak from the heating tube. Crude oil was observed inside the heating tube and on the ground surface near the tank. The oil was not impacting any bodies of water. Several photos were taken of the well and tanks. A GPS reading was taken at the wellhead and tanks. Note: The well status at the time of inspection was (ND) Not Drilled. Time of inspection: 11:00 AM.
