1418975971 2016-07-27 Production Wells No Violations This inspection was conducted as a status check. I was accompanied by adjacent landowner Mr. Stan Blakley. The well was situated on the edge of Lost Run Creek which is a tributary of Rocky Fork Creek. The well appears to consist of 8 casing and 2 tubing. The 8 casing was welded to the tubing. The top of the casing is approximately 8 feet above the base of the creek. A valve was observed on the tubing and did not have any rods in the tubing. No production lines were observed coming from the well. The casing is causing stream debris to dam up. No leaks were observed. I took (7) digital photos of the well and creek. The landowner will be contacted and informed of inspection. Time of inspection: 2:30 PM. Weather: 86*F, Sunny
1552319161 2019-03-07 Production Wells No Violations I conducted an inspection on this well to refer it to the Orphan Program. The well consists of 8 inch casing and 2 inch tubing. The top of the casing is approximately 8 feet above the base of the creek. The tubing is welded to a collar on the 8 inch casing. The tubing is equipped with two valves but does not have any production lines connected to the well. The well is situated within Lost Run Creek which is a tributary of Rocky Fork Creek. I observed water leaking from below the collar on the 8 inch casing. The water was tested for chlorides with a Quantab titrator test strip. The test resulted in a reading of 0.4 Quantab units which equals less than 32 ppm for chlorides.
