The gate does not have a urban id. A mechanical separator and plastic tank is outside of the fence. There are two separators and two tanks, one for LEDCO #1 and one for LEDCO # 5. Plans have been made to fence in the production equipment and place an id on the gate. The well is not fenced in yet. No id at wellhead. Id will be on the fence. Pit location has been filled in. pit site has settled some. is allowing water to pool. Site needs to be graded then seed and mulch applied.

The stake is 27 (feet) from a railroad spur. The rail spur is damaged and is not being used at this time. I have requested a letter from LEDCO stating that the railroad spur will not be used.

Separators have been installed for the LEDCO # 2 & 5, Plastic tanks have been installed for brine. The Separators and tanks are to be fenced in with an extension of the LEDCO # 2 fence that is around the well. A separate fence will be installed at the LEDCO # 5 wellhead. The drill pit has been closed. Site has been graded. Soil has settled at pit location. A pile of top soil is on location and is to be used in filling in low area. Well is equipped with tubing. Well is connected to Separator and tank. Well is connected to production line. Well does not have identification yet. LEDCO #2 id is on fence.

1474303797 2009-09-11 Drill / Deepen / Reopen No Violations Surface cement job. TD 305 . 120 of 16 64lb Conductor. 272 of 26lb 7 casing in a 12 3/8 hole. Pin Point Drilling Rig out of Beckley, WV. 7 ABC extinguishers checked during inspection all had current inspection tags and were full. Flow line to pit was not anchored down. I told Jeff Price (NYTIS Drilling Supervisor) to chain the flow line down in at least two areas. I also told Jeff to completely bolt the flanges on all sections of flow lines. Some flanges only had a bolt in every other hole( they are 10 bolt flanges). Well achieved good circulation of water to surface. After circulation bentionite and flake were circulated 5 sacs of mud mixed. Bentionite circulated to surface. Cement was pumped and cement circulated to surface.
1543425010 2010-06-14 Production Wells No Violations No fencing around the separators and plastic tanks for the #1 and # 5. I called Jeff Price and he told me that they are holding off the fence until after they plug back the #1. I asked for a letter stating a time line for the plug back and the fencing.
1841141314 2009-04-08 Administrative Inspection No Violations Received a call from Jeff Price (NYTIS) He wants to set 7 inch casing through Berea and cement to surface. The 7 inch will be the surface casing. I talked to Steve Oppritza ODNR Permit Geologist. Steve said that setting 7 surface casing bellow the Berea is ok as long as NYTIS understands and accepts the risks of contamination and the liability of such a action. I called Jeff back and informed him of their liability for any contamination. Jeff will discuss this with NYTIS.
1846878828 2010-07-20 UrbanProduction Wells No Violations Fence has been extended around tanks and separators. No identification on gate. A fence has been placed around the well. It consists of 10 foot sections of premade fence (kennel type) that is configured in a square with three strands of barbed wire and a locked gate. The fence is not secured to the ground with posts. The fence is not permanent with out securing it to ground. No identification on well. I called Jeff Price and he will post identification on well and gate. He is going to secure the fence to the ground.
2012242533 2013-04-02 UrbanProduction Wells No Violations Jon Scott took me to this well site and other LEDCO lease wells to inspect good examples of urban township production wells, urban well identification signs, fence construction, etc. I took pictures of signs and fence to use as good examples future urban well inspections. This wellhead is in its own fenced-in area. Tank battery used to collect brine before entering the gas production line is in the enclosure around the LEDCO #1 (API # 3408720645)
