0178362337 2009-05-11 Administrative Inspection No Violations Called Mark Moody today 5-11-09 at 1:30 pm. I told Mark of the slip and the brine seep. We also discussed the gas leak at the well head. I told Mark of the similar brine leaks on other horizontal wells and asked that the surface casing be exposed. Mark is going to follow up with the field personnel and get back to me.

Mark Moody (Great Lakes) is going to send an request for extension on leaving pit open for longer than 30 days. They are still planning to drill second well soon. The site is such that closing the pit and digging a new one would be difficult and cause damage to the site. Pit has some good mulch and grass on it now.

Some minor issues are present but nothing to prohibit passage of preliminary urban restoration. There is no tank battery associated with this well.

The well is still leaking ( bubbling up around wellhead ) Dale said that he will dig up the cellar after he blows down the well. This will allow a better look at the leak. I told Dale that he needs to drain the standing water on the site and that he must drain it through a straw bale sediment catch.
0558502275 2007-03-14 Drill / Deepen / Reopen Violations Noted No effort to comply with NOV # 1551545387. Pit is still full and now the Safety fence is down on 75% of the pit perimeter. A diversion ditch has been dug around the side of the pit to divert surface water away from pit. No erosion controls are in place on the diversion ditch. Pipe line work has been done recently down to the well. I called Mark Moody 11:30 am on 3-16-07 and went over this NOV with him. Mark said that Ross will take care of the pit levels and safety fence. They are going to produce this well and see how it does before any more work is done to the well. Restoration is waiting on dry weather.Mark said that the leak at the well head is coming up from the cement job. Between the 7.5 and the conductor.

0800046008 2008-01-08 Production Wells Violations Noted Id at well is too small to meet urban requirements. Sign is 11 by 7.25 and the lettering is .5 tall. A 4 line has been connected to the well and a 1.25 yellow plastic line has been ran to the well and not connected. No tracer line or pipe line identification was installed with this 1.25 line. No fencing has been installed at the wellhead. The access road has some small slips beside it that have filled in the diversion ditch and blocked a culvert. This diversion ditch drains a steep section of the road and is critical to the erosion control efforts on this site. The access road has a gate that is locked and has no identification. I am requesting a key to the gate per 1501:9-9-05 (E) (3)
0810323960 2009-05-19 Production Wells No Violations Meet with Rick Keplar (Range Resources) and his contractor. The slip has moved since my last inspection. Rick agreed to a 2nd meeting with Lawrence County soil and water to discuss how to fix the slip. Meeting next week. I contacted soil and water they will get back to me on a day next week.

The gravel channel and well are not effected by the slip at the time of inspection. Frozen water was observed at the base of the slip. The access road has a large water bar installed at the top of the steep portion of the road. Took pictures of the slip.

Dave Cumberledge (Great Lakes Southern Area Production Supervisor) Was on site to witness the leak and to come up with a solution to the problem. The pits have been filled in and site has been graded.

The slip is starting to impact the hillside. The id has not been posted at the gate. The wellhead still has the small id as described in NOV. A regulator and gas meter have been installed for house gas. This whole assortment of equipment is being supported by an upside down plastic milk crate.

1178117362 2007-05-02 Drill / Deepen / Reopen Violations Noted Dale Ross is pumping the pits down, however there is some pit water that has been pumped into a shallow ditch near the wellhead. This water is the yellow color of the pit water and has some brine in it. The ditch is not lined so the pit water is going into the ground. I called Mark Moody (Great Lakes) and told him what is going on. Mark is going to call Dale Ross. I left a note for Ross on the lease road gate tellig him to pump out the water in the shallow pit and to fill it in now.

1183009061 2007-07-18 Administrative Inspection No Violations Called 330-877-6747 at 1:35pm on 7-18-07 and left a voice mail message for Mark Moody ( Great Lakes) I informed him of the errosion on the lease road and the lack of drainage on the flat area around the wellhead. The Run off is causing erosion on the site.
1184173046 2007-07-09 Preliminary Restoration No Violations Good vegetative cover on site. Grass is growing. Lease road needs more work done to it. There are not enough ditches to divert the water off of the road. As a result of the water on the road there are channels eroding in the road. There are slips along the road cut that need to be fixed. Silt is building up at bottom of the lease road near the last culvert. The new culvert that was installed on the bend in the road is causing erosion at the point of discharge. The gas leak around the 7 can still be heard and smelled at the well head. A lack of drainage on the site is causing erosion to move off site. The runoff has turned the old logging road into a stream bed. The reclaimed area at the wellhead is 600 feet by 180 feet. This is not including the lease road area.
1212597699 2008-04-30 Production Wells No Violations No Urban Id on Access Road Gate. Gate was not locked. I requested a key to the lock and have not received one. The slip on the access road has got worse. Trees are now blocking the access road. This blockage will hinder an Emergency Responder from getting to the wellhead in a timely manner. The diversion ditch is completely filled in. Surface water is eroding the roadway. Wellhead has the same little id. No work has been done since the last inspection.
1219233327 2008-07-23 Production Wells No Violations Access road gate has urban identification posted. The slip beside the access road has not been fixed. Access road is blocked with three downed trees from the slip. A new id has been placed at the wellhead this id is small with 1/2 inch lettering.

High End quantabs were used 1st in excavated area near pit side read 9 on the strip. 2nd in scald area near wood line read 8 on the strip. Took photos.
1310043366 2009-03-31 Production Wells No Violations Access road has been cleared. Slip along access road has been worked on and diversion ditch is now functioning properly. Site needs seed and mulch applied to establish vegetative growth. An Inspection of the wellhead revealed a new slip near the drill pit location. Water is seeping out of the ground at this slip site. Deer tracks and salt smell at seep suggested brine. I tested the water with a High Range Quantab the results were 7.6 on the strip. Which is 4477 ppm Cl. Pieces of black plastic pit liner were exposed due to the slip. A sample of the brine was sent to the Lab. Seep sample site GPS N38.47311 W082.49854. I called and Talked to Mark Moody about the seep and the slip. Mark is going to send field personnel to location. They will call to set up a meeting. Fix slip site and determine where the brine source is. Schedule a meeting in field to discuss plans.

George is installing a gravel drain at the base of the hillside to intercept the water and divert it away from the well site. He has dug a trench with fall and has a layer of gravel then felt then gravel with a 4 PVC pipe in the center.
1743074816 2007-08-07 Preliminary Restoration No Violations Good vegetative cover on site. No identification at wellhead or on access road gate. Access road is very steep and does not have proper drainage controls installed to handle the watershed that the access road is exposed to. The diversion ditch needs to extend all the way to the top of the road. A series of water bars need to be installed on the access road to prevent erosion. There are two small subsidence areas on the flat wellhead site. Probably caused by buried trees and debris. Mark Moody told me that Dale Ross and a Great lakes representative would call me about these problems. No one has called as of today. I will call Mark Moody again.
