The stream is channeled to the center of the crossing and will be accessible during normal flow patterns. Large rocks are placed on the sides of the crossing to help stabilize the stream bank.
0517241961 2007-07-18 Administrative Inspection No Violations Talked to Rick Green (EOG) at 11:19am on 7-18-07 about the lease roads stream crossing that they worked on 7-9-07 the crossing has been built up 3 feet and has no culverts and is made out of sand and rocks. The crossing will not be able to with stand any real flow in the creek. Rick said that he cannot spend anymore money bringing in out of town contractors to work on the crossing. I suggested using the contractor that they are using on the Humphrey reclaim.

0768017017 2007-07-02 Administrative Inspection No Violations I had a meeting with Rick Green (EOG) , Greg Mills, Gary Green ODNR. On site to discusss stream crossing and road erosion. Rick said that he will get a contractor on it in a week and will address the problems.
1121594605 2007-07-09 Production Wells No Violations I received a call from Rick Green at 12:20pm that his contractor from Marietta moved on site at 9:00am. I was in South Point and drove to the site in the hopes that I could catch the contractor at the site. The contractor was gone when I arrived at the site. The contractor did not do what Rick Green agreed to do, which was to fix the stream crossing by putting a shallow bed of stone and concrete in the bottom of the stream bed. They were going to cut back the approach to the stream and make it more accessible. A sand bar in the middle of the stream was to be moved to the side of the stream where the bank is slipping. The contractor used the sand bar to build up a 3 three foot dam across the stream in stead of following the plan that was agreed on at a meeting on site on 7-2-07 with Rick Green, Greg Mills, Gary Green, and Jon Scott. The contractor did do some of the water bars that were agreed on. No water bars were installed past the second gate on top of the hill. I called Rick Green at 4:00 pm on 7-9-07 and informed him that the work was not satisfactory and it needs to be fixed to the original plan that was agreed on at the meeting on 7-2-07.
1182799586 2007-06-15 Administrative Inspection No Violations Took Greg Mills to look at lease road erosion and stream crossing. Lease road needs water bars every 50 feet on the steep slope and then every 100 feet on the rest of the road. Stream crossing needs to be reworked to allow flow in the center of the channel. Rocks and sand needto be placed on the side of the bank that is slipping. I will schedule a meeting with EOG to go over plans to fix the road.

I told him that I have talked to Jim Ross and that he could fix the crossing by manipulating the rocks and concrete that is in place to allow the stream a clear channel in the center. This channel will not impound the stream and should help divert the flow away from the stream bank that is eroding. Nathan agreed to follow up on this plan and will call me in a few days.

1369149557 2013-05-14 Production Wells No Violations This is a pneumatic lift wellhead. There is a 9-1/2 inch surface casing rising above ground surface. Rising above the cap on this casing is 4-1/2 inch casing ending in a t-fitting. Out the sides of this fitting are two 2-3/8 inch pipes. Above the cap on this fitting rises 1-7/8 inch production tubing to a master valve. This tubing extends above the top of this valve into another t-fitting. From the side of this fitting is another 2-3/8 inch pipe. Above this fitting a 1-7/8 inch production tubing rises to another t-fitting. On top of this fitting is the rabbit tube. The tank battery consists of 100 barrel steel tank for brine storage and a tall liquid/gas separator. There is no containment around the tank or separator.
