0517240926 | 2008-01-07 | Production Wells | No Violations | Small oil spill less than 1Bbl. A valve was opened on a tank and the oil leaked in to the dike. The dike drain was open and the oil drained out of the dike and down a path to a flat area. The oil was contained with in ruts in the path. The leak was noticed on Sunday 1/6/08. I received a call on Monday 1/7/08 from Rick Green informing me of the leak and their clean up efforts. I arrived on site on the 7th and the oil had been contained and absorbed and collected in a pile with plastic under it. The dike and path had absorbing material on it to collect any left over oil. They are going to compost the contaminated soil. |
0604762884 | 2017-05-22 | Production Wells | Violations Noted | On day of inspection, I accompanied inspector Matt Fulk. Inspection revealed tank battery that had brine water standing in dike. I dropped three quantabs, they ranged from 800-1634 ppm. Spoke to a company representative about cleaning and remediating inside dike. |
0990727344 | 2017-06-21 | Production Wells | No Violations | The dike was free of chlorides, there was a small amount of water in the dike. This water was tested with quantabs and revealed no chlorides. Grass was growing in the damp area and there were tadpoles in the water. |
0997602722 | 2014-02-02 | Production Wells | Violations Noted | Jim Ross, contractor for Enervest Operating LLC, contact me Saturday afternoon, yesterday, that they had experienced several oil leaks from pipelines thave oil from the ERP field to tank batteries positioned throughout this field. One of the pipelines involved connects this well and three other wells to a tank battery that contains two 210-barrel and one 50-barrel tanks, 1300-feet north of this wellhead. The leak occurred in the containment area of the tank battery next to one of the large tanks. The entire field operation was shut down during the sub-zero temperatures to prevent any possible leaking during this weather event. Leaks started when the wells went back on line after the temperature got warmer. The operator was aware of the situation before it created too much contamination at the surface. He shut in the impacted pipelines and repaired the damaged pipes. The operator has spread adsorbent material including peat moss and Millers Supply Biosorb on the impacted area to immobilize and solidify any free oil. The contaminated soil will be staged inside the battery until the operator decides to remove all the contaminated soil staged in other areas All contaminated soil at this site will then be manifested, put into Rumpke roll-off containers and disposed in Rumpkes landfill east of Jackson, Ohio. |
1018856607 | 2014-04-02 | Production Wells | No Violations | Upon inspection, the wells in this field were no longer shut in. The areas that were previously contaminated have been excavated and staged in a common stockpile (covered with a tarp) to be hauled off to the Rumpke Landfill near Jackson. The affected areas still showed oil sheen on the surface and in puddles. Enervest Well Tender Dustin Wilson showed up during my inspection, and I explained to him that the stockpiled soil needed to be disposed of as soon as possible, and I requested them to spread more peat moss or other oil absorbent material onto the affected area(s) to help soak up any more free oil. |

1910767763 | 2008-01-16 | Production Wells | No Violations | Follow up inspection of the oil leak cleanup on the 7th. Site looks good. Slight oil seeping from some shale. The oil is being contained by an earthen dike and absorbent material is being applied. |
