0329813760 2014-02-02 Production Wells No Violations FOLLOW UP TO NOV # 1391432378 Jim Ross, contractor for Enervest Operating LLC, contact me Saturday afternoon, yesterday, that they had experienced several oil leaks from pipelines that move oil from the ERP field to tank batteries positioned throughout this field. One of the pipelines involved connects this well to a tank battery, that contains three 210-barrel and one 50-barrel tanks, about 1200-feet west of the wellhead. The entire field operation was shut down during the sub-zero temperatures to prevent any possible leaking during this weather event. Leaks started when the wells went back on line after the temperature got warmer. The operator was aware of the situation before it created too much contamination at the surface. He shut in the impacted pipelines and repaired the damaged pipes. The operator has spread adsorbent material including peat moss and Millers Supply Biosorb on the impacted area to immobilize and solidify any free oil. The contaminated soil will be scrapped off the area around the leak dumped into a containment container constructed from a 210 barrel tank. The tank will then be covered with plastic until, weather permitting, the soil will be manifested, put into Rumpke roll-off containers and disposed in Rumpkes landfill east of Jackson, Ohio.
0485287136 2014-04-02 Production Wells No Violations Upon inspection, the wells on this field were no longer shut in and the pumping unit was operating upon my arrival. The well has legible and correct identification. The area that was previously contaminated has been excavated and staged in a common stockpile (covered with a tarp) to be hauled off to the Rumpke Landfill near Jackson. The affected area still showed oil sheen on the surface and in puddles. Enervest Well Tender Dustin Wilson showed up during my inspection, and I explained to him that the stockpiled soil needed to be disposed of as soon as possible, and I requested them to spread more peat moss or other oil absorbent material onto the affected area(s) to help soak up any more free oil.

1469692519 2018-08-08 Production Wells Violations Noted My inspection revealed the well consists of a pumping unit that pumps into a common tank battery. The wells identification was not accurate. The identification listed Entervest as the owner. The identification needs to be corrected at the well and the permit number needs to be placed on the tank battery. I met the pumper on site and he said the identification would be corrrected.
1497962706 2017-06-20 Production Wells Violations Noted Driving by the location I saw straw on the ground outside of the dike. I pulled in to take a closer look and found there to be an old release that was cleaned up. However there was standing oil outside the dike on the south side of the dike that will need cleaned up it seemed to originate from the dike drain. I will stop at the shop and have the employees get the area cleaned up.
1638497303 2018-09-17 Production Wells No Violations The reason for this inspection was to determine if the compliance notice that was issued for pollution and contamination could be written off. The compliance notice is satisfied by no pollution or contamination reaming inside the tank battery and outside the tank battery. Vegetative cover was observed outside the dike where the spill was located. I took GPS coordinates at the location of the spill.
