0473266599 2013-09-27 Plug / Plug Back No Violations Benny Alteir, Alteir Brothers, was able to get a cement company, The Wells Group LLC, to finish the top off the surface casing plug adding 29.26 sacks of Class A neet cement to finish the surface plug. They tagged the plug from yesterday and it had fallen 97-feet below the surface before it stopped. When this plug was set it only dropped two inches from the top of the surface casing over a one hour period and then stopped.

I requested that they put my name on their contact list for any future well related activity. I talked to Randy Rodgers the Deputy on site for the accident. I received a copy of the incident report.

0951941028 2010-08-09 Production Wells No Violations Received a call from Jim Ross at 5:30 am on 8-9-10. Jim told me that a truck had ran over the Clay well. He told me that the Sherriff and Fire Department was on location. I called the Lawrence County Sherriffs office to verify this information. I then called South Region Supervisor Robert Stonerock and Rich Jones USFS and left messages on their voice mail. I arrived on location at 6:30 am. Deputy Randy Rodgers and Brian Depriest (Aid Twp. Fire Department.) were on location. A GMC pickup truck was on top of the well. The truck had hit the tubing and broke the valve off the well, causing uncontrolled natural gas to flow from the well (LEL 72 on gas meter). After the gas blew down, the truck was lifted off the well. I coordinated with The USFS (well owner) by getting them contractor contact information and detailed what actions need to be done to contain the well. I met Rich Jones USFS on location at 10:00 am. I also met with USFS Officer Babatunde Oyewole in the field and later that afternoon at the Jackson office to share photos and information that I collected at the scene.
1220900378 2010-10-15 Administrative Inspection No Violations Received a call from Rich Jones USFS Re: repair status. Their contractor was able to install packing above the slips and then re tighten the hammer ring. Rich informed me that this stopped the major gas leak. There are a few small leaks around the nipples on the annulus of the wellhead. Rich will have the contractor repair those leaks as well.
1229186484 2010-10-08 Production Wells No Violations Meet with Rich Jones USFS on site to discuss repair options. The main priority is to stop the gas flow from the wellhead. Best case the hammer ring needs to be tightened. Worst case the wellhead is damaged and needs to be replaced, in this case I told Rich that the tubing would have to be removed from the well and a new wellhead installed. I advised Rich that they will want to keep the tubing for use during the upcoming plug job.
1287145311 2010-10-07 Production Wells No Violations Received a call from Rich Jones USFS, Re: Clay well was observed leaking gas by a Wild Life Officer. I responded and verified by inspection that the wellhead is leaking gas. The top of the wellhead must have sustained additional (undetectable) damage due to the recent vehicle accident. Gas meter recorded no LEL. It was windy and I was standing up wind of the well. I held the gas meter directly over the stream of gas and no LELs were measured. I could hear and feel the gas flow from the wellhead. I contacted Rich Jones and he is arranging a contractor to give an estimate to fix the leak. I reminded Rich of the need for the USFS to get registered as the well owner.
1331565414 2010-08-11 Production Wells No Violations Follow up with Rich Jones USFS and Francis Brothers , and Jim Ross on location. Plan to cut off tubing and stab with tool to retrieve 1.5 tubing and pull up to next collar. Then install new valve. Work is scheduled for Thursday morning.

1373034028 2013-07-03 Production Wells No Violations Morgan Button MRI 2 and I responded to a report of a leaking gas well owned by Wayne National Forest. We met Scott Means USFS on location. The gas detector showed LEL levels ranging from 9 to 24. The wellhead slip ring was leaking gas. Jim Ross (A local contractor ) was contacted. Jim checked the pressure on the annulus, it was less than 100 psi. Jim blew down the well and replaced the rubber washers, applied grease and tightened the slip ring. All valves on the annulus and the master valve on the 1.5 tubing are closed with a bull plug installed. There were no leaks observed. An orange fence was placed around the well.
1380891379 2013-09-25 Plug / Plug Back No Violations I arrived at 0730 hours on location and Jon Scott arrived at 0745 hours. Jim Wood was on location already. Benny Altier of Altiers Brothers, that received subcontract to plug this well for the Wayne National Forest, arrived at 0815 hours. Their service rig and other equipment was already at the location. Bennys crew began working on the wellhead and rigging up their mast on the service rig. Formation Cement arrived about 0900 hours and began prepping for the first plug, Clinton sandstone. Benny elected not to pull the 1-1/2 inch production tubing against my and Jon Scotts recommendation. So we were not sure of the depth the end of the tubing was located and other considerations such as a bridge plug etc. Formation tried to established circulation through the tubing and up the 4-1/2 inch production casing to surface. Pumped 100 BBls of fresh water and had no circulation returns to surface. Fluid level was estimated at 150 feet below ground surface in the 4-1/2 inch casing. Formation started at 1230 hours to run cement plug through the 1-1/2 inch tubing spotting the plug from 3315 to 2821-feet. With 35 sacks of Class A type cement with cottonseed hulls mixed with cement slurry initially pumped to bottom. After placement of this plug the 1-1/2 inch tubing was removed and tallied to verify the depth and that indeed the Clinton sandstone perfed interval of 3168 to 3160 feet was plugged. The top of the Clinton was reported at 3158-feet on the completion ticket. The tubing removed was tallied at 3175.2-feet in length, 98 joints. JT Wireline Services arrived to run the Cement Bond log (CBL) to establish the optimum spot above the top of cement on the outside of the 4-1/2 inch casing to bust a collar and then remove the freed pipe from the well. According to JT Services they tagged and confirmed that the top of the Clinton cement plug was at 2821-feet. The CBL located the first free collar above the cement on the outside of the 4-1/2 inch casing at 2514.7-feet.
1422431662 2010-08-12 Production Wells No Violations Francis Brothers rig on location. Rig up, Cut tubing off to allow spear to stab tubing. Slight gas flow from tubing. Disassemble slips and open annulus valve. Stab tubing with spear. Pulled tubing up to next collar. Reassemble slips and close annulus valve. Install 1.5 by 2 change over and new 2 valve. Shut valve. Rig down.

1547915934 2009-02-23 Production Wells No Violations Well is disconnected and shut in. No production equipment. Jim Ross requested this inspection in order to determine who current owner is. I contacted the USFS and confirmed they are the well owner. They plan to plug the well soon.
1759347174 2013-07-03 Production Wells No Violations Scott Means, USFS Representative contacted me via email in the afternoon of 7/2/2013. I called him in the morning after I read the email about the situation. He explained to me the status of the well and that it was leaking loudly from the packer assembly between the cap on the intermediate casing and the 1-1/2 inch tubing. He told me he had placed a fence around the wellhead, but nothing more and wanted to know what he should do further. He was concern about the potential hazards if the well was left in this condition until they got approval to hire a contractor to plug this well. I inquired about any houses, farm animals, ATV trails, etc. that might be impacted by this leak. I also noticed the location of this well was in a secluded valley which might present a problem if there was no wind to disperse the gas allowing the gas to build up to explosive limits. We agreed to meet at the wellhead. I contacted Jon Scott who was familiar with the situation of this well from prior inspections. He was in the neighborhood and agreed to meet us at the wellhead to help in the assessment. Once we evaluated the situation we all felt this situation was too dangerous to leave without some effort to shut it in and possibly repair the leak. Jon Scott called Jim Ross, contactor for Enervest Operating in Lawrence County. Mr. Ross showed up, assessed the situation, and agreed that the problem needed to be addressed immediately. He offered his services to Scott Means to correct the problem on a labor and material charge. Mr. Means contacted his office in Pedro, and got approval to use Mr. Ross to mitigate the problem. Mr. Ross determined that there was only 90 psi shut in pressure and felt he could bleed the gas off the wellhead. He obtained a used packer from his equipment in Greasy Ridge ERP, bled the gas pressure off the wellhead, pulled the dry rotted packing and replaced it with his packer. While it took some time to accomplish all that took place, it was a good

1905077944 2013-09-26 Plug / Plug Back No Violations Arrived at location and before any motors started at the location checked out the status of the well. Apparently there was a gurgling sound coming from the well. No gas emissions from the open wellhead. The next plug to be set was the Berea Sandstone at 1280 to 1276-feet. Pulled 4-1/2 inch casing to a depth of 1411.3-feet. Benny removed 1103.4 feet of 4-1/2 inch casing, 29 joints total. Fluid level in the 4-1/2 inch casing was still at 150-feet. They bullheaded 65 sacks of class A neet cement to cover 50-feet below and 100-feet above the top of the Berea. After placement of this plug the well was still gurgling. Elected to remove the remaining 4-1/2 inch casing. The plan was to use the 1-1/2 inch tubing to tag the top of the Berea plug to confirm it had not fallen. One concern was that plug may have fallen. The other is the plug has not had sufficient time to completely set up based on the hardening of the sample collected when the plug was being injected into the casing. The tubing did not stop as it was lowered until it reached a depth of 1352-feet, approximately 70-feet below the bottom of the Berea. Benny raised the tubing to approximately 1300-feet and Formation spotted another 53 sacks of Class A cement raising the top of the Berea plug to 1100-feet. Benny the pulled tubing to 715 feet in order to set the 100 feet-in/100 feet-out plug at the bottom of the 8-5/8 inch surface casing at 540-feet, They set 75 sacks to set the top of the plug at 440-feet. An attempt to set the surface plug was made but it did not stay at the surface. Formation set 37 sacks for this plug It kept falling to almost 100-feet below the ground surface. The gurgling sound may have been from a hole in the 8-5/8 inch surface casing keeping fluid levels at a constant depth after the 4-1/2 casing was ripped. Shut down for the night.
