0119922043 2018-04-02 Orphan No Violations I inspected this well location pursuant to a request by a real estate agent, regarding the presence of an unregistered well at the residential property. At the time of my inspection, I found the following: an estimated 6-inch diameter wellhead with 4-bolt, square flange lid, within a 4 by 4 concrete vault structure. There had been a wooden lid over the vault, which had become severely degraded and broken down, with portions collapsed into the vault. There was audible leakge of gas from some undetermined portion of the well or associated fittings. There were at least two, distinct domestic flow-lines attached to the well, which reportedly supply both the house and the rental apartment over the garage.
