1481833487 2016-10-17 Administrative Inspection No Violations I was contacted by oil and gas well contractor Paul Mihalik who was contracted by the homeowner to perform consultation and repair work on this well. According to Mr. Mihalik, he found and repaired a leaking fitting at the well, but there may have been some gas leaking from outside the casing. I communicated these findings to the homeowner and advised that they would need to file for registration, since the well was not documented. I subsequently submitted registration forms to the homeowner for their review and submission.

2061149445 2016-10-14 UrbanProduction Wells No Violations I inspected this well location pursuant to a call from the landowner regarding a leaking well near the house. At the time of this inspection, I observed the following; a circular, masonry vault structure, approximately 30-inches in diameter with a wooden lid. Located within the vault was a 6-inch diameter wellhead casing with 3-bolt lid, and 1-inch domestic supply and regulator and gate valve which was closed. I recorded 50 to 60% LEL within the vault, which dropped to below detectable levels beyond the vault. The well and vault structure were located within 10 feet of the exterior wall of the house, with a wooden deck structure over the well. The owner reported that a contractor performing work at the house noticed the gas odor near the well and observed a leaking fitting at the wellhead. The gate valve at the well was closed by the contractor pending evaluation. I performed a survey using hand-held gas meter in areas around the well, including the house interior and found no detectable readings anywhere outside of the vault. I advised the homeowner to consult with an experienced oilfield contractor to evaluate the well and provide consultation and maintenance. In addition, I discussed requirements for documentation and permitting for a presumptive exempt-domestic well. We agreed that I would follow-up with the landowner after they had the well evaluated.
