0014378050 2017-03-10 Administrative Inspection No Violations I contacted a memeber of the church leadership council (Norm Alber) to inquire about the on-going efforts to return the well to compliance with requirements for registration, insurnace, and bonding. Mr. Alber indicated that efforts to get bonding and insurance are continuing. We agreed that I would check back wihtin 2 weeks.
0162278052 2017-11-21 Production Wells No Violations I inspected this well location as part of on-going efforts to establish proper registration and ownership. The Painesville Baptist Church which owns the property on which the tank battery is located, has claimed ownership and has been utilizing gas from the well for heating the church for many years. I had previously met with the leadership council of the church and advised about the registration requirements. At the time of this inspection I had noted that the well ownership had not been transferred. I contacted Jeff Sell with the church committee and he reported that the paperwork was submitted as of 11/24/17.

This well had previously been unregistered but utilized by the Painesville Baptist Church. At the time of this inspection, the enclosure gates at the well and battery were locked. Fittings and connections were secure. I noted that the well remains listed as Historic Owner. I contacted ODNR Surety and Bonding regarding registration. Cynthia Marshal reported that the insurance policy was incorrect and needed attention. I contacted Jeff Sell, with the church, and a response was pending at the time of this report filing.

The Painesville Baptist Church which owns the property on which the tank battery is located, has claimed ownership and has been utilizing gas from the well for heating the church for many years. At the time of this inspection, I found the wellhead pump jack system and tank battery intact and in active use with a connection to the church. I contacted Cynthia Marshal at ODNR Columbus regarding status of ownership documents. She reported that there had been ownership paperwork received but that the insurance certificate had incorrect information and had to be corrected. I contacted the church and a response was pending at the time of this report filing.
1474387647 2016-09-19 UrbanProduction Wells No Violations I met with the leadership council of the Painesville Baptist Church located at 10801 Johnnycake Ridge Road. The church claims ownership of this well and we met to discuss ownership requirements. At the time of this meeting, I presented guidance on ownership registration, bonding, and insurance requirements. The parcel on which the well is located is owned by others, and was undergoing a possible sale. The church shared historic documentation regarding production data and deed access easement to the well. I advised the leadership council to begin evaluating ownership, bonding, and insurance issues and I would discuss with permitting and well ownership at ODNR central office. We agreed to reconvene via telephone later in the week.
1481646650 2016-12-07 UrbanProduction Wells No Violations I inspected this well location pursuant to a request from the landowner of the property (Mr. Donald Clark) on which the well is located. I had received a call from the landowner regarding a possible dispute over ownership of the well and associated subsurface mineral rights. I met with the landowner at the house and I inspected the well and surrounding areas. At the time of this inspection, I observed the following: a wellhead with pump jack system in a chain-link enclosure and tank battery in separate enclosure. The battery was comprised of one 140-barrel steel tank and separator with associated supply connections and fittings. The enclosures were locked and secure. There was no identifying signage. I advised the landowner that the 1st Baptist Church of Painesville, which is south and adjacent to his property, is claiming ownership of the well, and is in the process of completing registration paperwork for ownership. I provided Mr. Clark with contact information for the church and for ODNR Oil and Gas Division Permitting, and advised him to utilize the contacts for questions regarding well ownership.
1481662083 2016-12-07 Administrative Inspection No Violations I contacted James Richner, representative of the 1st Baptist Church of Painesville to discuss the progress and status of the on-going efforts by the church to establish proper ownership and registration. I had met previously with the church leadership council and provided direction and assistance towards establishing proper permitting. At the time of this call Mr. Richner indicated that the church was finalizing documentation as required. We agreed that I would check back with him by the end of the month to confirm completion of the material.

1547478373 2019-01-14 UrbanProduction Wells No Violations I conducted an inspection at this well location as a follow-up to complaint #5984, regarding a gas odor at the well. Upon inspection of the well site, no gas odor was present at the well or tank battery location. The well was equipped with a pumping unit and the valve on the casing was open. The well was fenced with a locked man door. The tank battery consisted of one 100 BBL tank. The tank battery was fenced with a locked man door. Legible identification was present at the tank battery and contained up-to-date owner information. The complaint log did not contain a phone number to contact the complaintant, Mr. Donald Clark. I then proceeded to go to the address at 10734 Lindmar listed on the complaint log, and discovered that Mr. Clark no longer resided there. The current inspection at this well location yielded no violations. Complaint can be closed.

1785887266 2018-09-21 UrbanProduction Wells No Violations I inspected this well location pursuant to a complaint from the surface landowner at the well. The landowner had reported smelling gas around the wellhead, which had started to occur recently. I notified the well owner about the complaint. Upon this inspection, I found an intermittent odor of natural gas near the wellhead, but at concentrations below the threshold for detection on my persoanl metering device. I did not note any apparent leaks, either visual or audible, wihtin the production system. The owner had indicated that the well service contractor was notified and would evaluate.
