The secondary containment is adequate. Gas is leaking around the wellhead at the ground surface. There has been no addition of a pop-off valve as had been previously recommended. The ID needs corrected in the placard.

The well and battery are located via a dedicated lease road adjacent to the referenced address. The wellhead and battery are located in a farm field associated with the farmstead. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or other problems. The load-out is plugged and secure but needs a lock. Evaluation of drainage and other surficial features is limited due to snow cover. I did not observe any evidence of impact relating to on-going production activity.

0812203236 2012-09-06 UrbanProduction Wells No Violations I inspected the referenced unit pursuant to a nearby landowner complaint of salt in their drinking water well. The landowner resides at 12275 Painesville-Warren Road. The wellhead is located 410 feet northwest from the complainants property line. The existing wellhead consists of a typical valve stem assembly with a mechanical plunger lift system. The well is in production. Gas pressures at the wellhead indicate 400 pounds per square inch (PSI) reading at the 4.5-inch casing, and 125 PSI at the production tubing. The 8.75-inch casing pressure reads 20 PSI. I observed occasional odors of gas but nothing was detected on the meter. I did not observe any other signs of leakage of oil or brine at the wellhead. The associated tank battery (TB) is located 75 feet southwest of the wellhead. The TB is comprised of one, 100-barrel (BBL) production tank, one, 50-BBL brine tank, and mechanical separator unit with associated valves and mechanical appurtenances. The production tank is fitted with a spark arrestor and a low-pressure relief valve. The perimeter dike appears to be adequate as well as the spacing within the diked area. The load out valve is not locked. I did observe some brine scalding outside of the diked portion of the TB. At the time of my inspection, all areas both inside the diked portion of the TB and around the production wellhead are free from standing water. In addition to the site inspection, I reviewed the well completion records for this unit, dated August 10, 2010. The records indicate that the 8.625-inch surface casing was set to a depth of 211 feet below ground surface (bgs) and there was no reported problems or lost circulation. The casing was cemented to surface with 105 sacks of Class A cement with 3 percent calcium chloride. The well was completed to a total depth of 3400 feet bgs, and production casing was set without incident or lost circulation.

I noted that the secondary containment dike drain is open, and I also noted that there is a lot of oil splatter and possible spill from the production tank. The lid on the tank is also open, I contacted Jesse Mulnar with Autumn Ridge and advised him to check on the issues.

Portions of the tank battery had become overgrown with wetlands grasses. I contacted the well owner and he had indicated that there was a pending sale of this production well, along with other wells owned and operated by Autumn Ridge.
