The tank battery secondary containment is adequate. I did not observe any leaks, spills, or other signs of contamination. The gates are locked.

I Called Jim from John D and requested the well head to be caged. The tank battery has the required HB-278 equipment installed on it and is in good repair.

The wellhead is a mechanical plunger lift. The battery is comprised of 2 140-barrel steels tanks and separator. Secondary containment, spacing, and signage is good. Process safety equipment (spark arrestor, carbon filter, and low-pressure relief valve) are present. The dike drain has been left open. I will contact Great Plains about the open drain. No problems otherwise noted.
1429709297 2015-04-22 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well and battery are located in a wooded area on the grounds of the Tripoint Medical Center. The well plunger lift is located in a chain-link enclosure. Connections and fittings are secure. The battery is located in a separate, locked enclosure which is east of the battery. At the time of this inspection the battery is comprised of one 100-barrel steel production tank, one 100-barrel steel brine tank, and separator. The load-outs are plugged and secure. Fittings and connections are secure. Production elements are in good repair. The secondary containment is intact and dry. Areas surrounding the well and battery are free from signs of adverse impact or erosional deficits. There is a small creek which traverses the property from north to south and lies between the battery and wellhead. Flow within the creek is normal and free impact.
1455033320 2016-02-09 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. The well and associated battery are located in a wooded area on the south side of Tri-Point Med Center. The well is directly accessed from the asphalt-paved parked lot of the hospital. Access to the battery is afforded by a short, unpaved access road from Auburn Road. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a chain-link enclosure. Fittings and connections at the wellhead were secure. The 8.625-inch casing is gauged and showing 80 psi. The battery is comprised of two 100-barrel steel tanks. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. Signage was present. I walked the areas around both the wellhead and battery and did not observe any signs of impact relating to the production activity. I noted the following safety issues: The enclosure gates at both the well enclosure and battery enclosure were not locked. The load-line valves were not locked. The owner must remedy the following by 04/08/2016: secure the enclosure gates and load-line valves with locks. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
1491828916 2017-04-10 UrbanProduction Wells No Violations I inspected this producing well as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required. At the time of this inspection, I observed the following: the access road to the battery, which is comprised of a gravel-paved road that enters from Auburn Road and travels 60 feet to the battery, was in good condition with no drainage problems or wash-outs. The well is accessed separately via the hospital parking and drive areas and has no dedicated access or lease road. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features.. I did observe the following maintenance and safety deficiencies; the load-line valves remained unsecure and require locks, the ID signage had not been updated to reflect the recent change in ownership. I contacted John Magnussen with Rockefeller Oil and advised him about the continuing defficiencies. He indicated that the items were being worked on as they are able. I contacted Richard Osborne Sr. with David Oil Co., and as of the date of this report, I have not received a response.

1924260096 2017-12-11 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves were not locked and secure. At the time of this inspection, I observed the following: the load line valves were not locked and the ID signage remains unchanged to reflect the new ownership transfer that occurred in 2016. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
