The owner must place compliant unit identification at the well within 30 days or be subject to additional enforcement. 0838758651 2017-09-07 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well and associated battery are located on a larger, rural parcel. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery. The various fittings and connections were secure and free from leakage or problems. The secondary containment was intact and dry. The tank lids were closed and secure. I walked the areas surrounding the wellhead and battery and observed no signs of impact to the natural greenscape.
0909832296 2009-01-29 Drill / Deepen / Reopen Violations Noted Drill depth 14, drilling on fluid. Pit liners were in good repair. It was noted during inspection that 2/3 of the pit was red. Operator had winterized pumps with off-road (#2) diesel fuel, 10 gallons. Fuel was in the pits floating on the ice and water. This was the fresh water pit, same fluid operator was using to drill CND hole. Operator was order to shut down and remove #2 diesel fuel from the pit. James and Jon, called in a brine truck to pump fuel from pit. Jon left site to pick up boom, for around the mud pump intake. Operator was able to remove majority of the fuel from the pits, boom will be sufficient to prevent fuel from going into intake. Called Tim Reiley Great Plains, on 01/30/2009, informed Tim that it was not acceptable to have any contaminates in the fresh water pits especially diesel fuel. Reminded Tim, that a small amount of fuel can contaminate thousands of gallons of fresh water. If any water wells are contaminated the company will be responsible to immediately replace water supplies. The company was drilling in the Ohio Shale at the time. Immediately shut down and clean off-road diesel fuel from the fresh water pit. Install boom around mud pump intake to prevent any residual fuel from contaminate fresh water intake.

1281366968 2016-01-26 Production Wells No Violations I inspected this producing well as part of routine inspection activity. Access to the wellhead and battery are afforded by a shared-use driveway. At the time of this inspection, I observed the following: A wellhead with pump jack system and tank battery with one steel 100-barrel production tank, one FRP 100-barrel brine tank, separator and metering run. I observed the back-pressure valve at the wellhead to be leaking gas. The remainder of the connections and fittings were secure and free from leakage. The secondary containment was intact and free from standing fluids, staining, or other signs of leakage. The load-puts were bull-plugged and secure. The access road was in good condition. I notified the well owner about the leaking fitting and they indicated it would be addressed wihtin the next 2 days.
1325169047 2011-12-27 Salt Water Injection Well No Violations Pop-off on the separator was discharging gas. Annulus on the well was static and there was minor gas bubbling on the outside of the 8.625 casing. The cables are off of the pumping unit. This well has not been converted to an injection well at this time. I called Jim from John D and left a message that there was gas leaking from the top of the separator.

The battery is comprised of one 100-barrel production and 1 100-barrel brine tank, and a separator unit. There is adequate secondary containment capacity and appropriate ID placard. The unit spacing requirements are met. The gas leakage observed previously at the separator and the wellhead was not observed at this time. No other problems or issues noted.
