At the time of this inspection, I observed the following: The noted deficiencies remained. In addition, the unit ID signage remained not updated since the change in ownership in 2016. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.

1406653862 2014-07-29 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 12/03/2008. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located down an access road which is a continuation of the residential driveway that serves 9546 Johnnycake. The access portion is graveled and in reasonable condition. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located via the same access road, and is 50 feet north of the wellhead. The battery is comprised of a total of 2 steel 140-barrel production tanks, and horizontal separator. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery but is not visible from the outside. I notified John D Oil about the sign.
1798792332 2017-06-16 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous inspection and associated Compliance Notice issued for missing locks and missing spark arrestor, low-pressure relief valve and carbon filter, and lack of exposed and gauged surface casing required under special permit condition. At the time of this inspection, these deficiencies remain. The company has pledged on a repeated basis to remedy the problems but have not acted as of this date.
1907359627 2016-02-26 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system and tank battery comprised of two 100-barrel steel tanks, and mechanical separator located in separate board-on-board enclosures. Enclosure gates were not locked or secure. The wellhead enclosure was observed to be partially torn down. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were not present. Fittings and c connections at the wellhead were secure and free from signs of leakage or problems. The 8.625-inch surface casing is exposed but not gauged as required under specific permit conditions. The owner must remedy the following by 04/01/2016: secure the load-line valves and enclosure gates with locks. Properly gauge the 8.625-inch surface casing as required by permit condition. Repair the wellhead enclosure. Fit the production tank with low-pressure relief valve, spark arrestor, and carbon filter as required.
