0243369519 2013-10-23 UrbanProduction Wells No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 2 100-barrel steel production tanks, and 2 separator units. The battery is shared with the #1264 unit. There are no enclosures. The load-outs are bull-plugged. The wellhead is not identified. The tank battery ID is good. No other problems noted. I will contact Great Plains about the ID placard for the wellhead.

0362194163 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located within the confines of the Little Mountain Country Club golf facility. The well is comprised of a mechanical plunger-lift system located at the fringe of the golf course fairway. Fittings and connections at the wellhead are secure. The battery is comprised of one 100-barrel steel production tank and one 100-barrel brine tank and separator unit in a diked area but not otherwise enclosed. Fittings and connections are secure. The secondary containment is intact and dry. Load-outs are secure. The areas surrounding the well and battery consist mainly of paved portions of the maintenance shop and associated driveway, parking, and equipment storage yard. I did not observe any signs of impact or drainage problems.
0844517586 2017-12-11 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves were not locked and secure and the surface casing was not gauged as required by special permit condition. At the time of this inspection, I observed the following: the load line valves were not locked and the surface casing was exposed but not gauged. The tanks must have a spark arrestor, low-pressure relief valve and carbon filter. In addition, the wellhead requires updated signage. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
0897806142 2017-09-25 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: missing locks on load-line valves and missing process safety equipment (spark arrestor, low-pressure relief valve, and carbon filter), and no gauge at the surface casing as required under special permit conditions. At the time of this inspection, the noted deficiencies remained. In addition, the well signage remains unchanged since the change in ownership that occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.

1423537153 2017-04-10 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous inspection and Compliance Notice issued for the following deficiencies: no locks on load-line valves, no spark arrestor, no low-pressure relief valves on tanks, no carbon filters, and no gauge on the surface casing annulus as required by special permit condition. At the time of this inspection, the noted deficiencies remained unaddressed. Repeated contacts to company represented John Magnussen have resulted in the following response: The deficiencies will be adressed as they are able.
1456512708 2016-02-26 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system located on the grounds of the golf course. A tank battery comprised of two 100-barrel steel tanks, and mechanical separators is located remotely at the maintenance garage. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were not present. Fittings and c connections at the wellhead were secure and free from signs of leakage or problems. The 8.625-inch surface casing is exposed but not gauged as required under specific permit conditions. The owner must remedy the following by 04/04/2016: secure the load-line valves with locks. Properly gauge the 8.625-inch surface casing as required by permit condition. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
