Status of the site does not warrant landscaping at this time. Well is in a gravel parking area. Tank battery is behind a hill and is screened by natural vegetation. Man doors need locked. Hatch on the 210Bbl production tank needs a seal. 210PRD tank extends above the allowable 3 feet above the fence. Pressure relief valves needs installed on the vents. Charcoal filter needs to be installed on the battery. Well is a rabbit well, well head is fenced. Contacted Jim from John D. and informed him of the needed work and gave him verbal compliance notice. |

0461588239 | 2018-02-13 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: The load lines were noted to be unlocked and unsecure, the fencing at the wellhead was broken down and unsecure, and the 210-barrel production tank was noted to be severely corroded and required replacement. At the time of this inspection, the indicated deficiencies remained. In addition, the ID signage remained unchanged and had not been updated to reflect the change in ownership that had occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results. |
0807693174 | 2017-05-05 | UrbanProduction Wells | No Violations | I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves, perforated production and brine tanks, and broken and unlocked fence enclosures. At the time of this inspection, I observed the following: The wellhead was comprised of a plunger-lift system with associated tank battery. The load-line valves were not secured with locks. The production and brine tanks with corrosion holes remain in the battery system. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages. |
0854128679 | 2012-01-10 | UrbanPreliminary Restoration | Violations Noted | FOLLOW UP TO NOV # 38395963 This well is equipped with a plunger lift system. The wellhead is fenced. The well and tank battery areas have been graded and seeded. The status of the site does not warrant landscaping at this time. The well is in a gravel parking area and the tank battery is behind a hill and is screened by natural vegetation. The man doors need locks. The hatch on the 210 barrel production tank needs a seal and the tank itself extends above the fence the allowable 3. A pressure relief valve and charcoal filter needs to be installed on the tank vent. I contacted Jim from John D and informed him of the needed work and gave him a verbal compliance notice. |
0869156874 | 2015-04-22 | UrbanProduction Wells | No Violations | I inspected this producing unit as part of routine inspection activity. The well and battery are located in a partially wooded area behind residential homes along Girdled Road. The well is comprised of a mechanical plunger-lift system in a board-on-board enclosure which is locked. Fittings and connections at the wellhead are secure. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure. Load-outs are locked. The containment dike is intact and dry. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable. |
1060431213 | 2017-12-05 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves were not locked, both stock tanks were severely corroded with numerous holes, and the wellhead enclosure was broken down . At the time of this inspection, I observed the following: there were no locks at the load-line valves, the rusted tanks remained on in place, and the wellhead enclosure remained broken down. In addition, the ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance. |

1346415093 | 2016-02-22 | UrbanProduction Wells | Violations Noted | I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system and tank battery with one 210-barrel, steel production tank, and one 100-barrel, steel brine tank. The well and battery were located in separate board-on-board enclosures. Process safety and air emission control devices (low-pressure relief valve, spark arrestor, and carbon filter) were present. I walked the areas around the wellhead and battery and did not observe any signs of impact relating to the on-going production activity . Access to the well and battery was afforded by separate short, dedicated, unpaved access road which was in good condition. I observed the following deficiencies: the wellhead and battery enclosures, and load-line valves were not secured with locks. The wellhead enclosure was partially missing. Both of the storage tanks were severely corroded with perforation holes in each which have compromised the integrity of these tanks. Production into the tanks must be stopped immediately. The tanks must be removed and replaced. The owner must remedy the following by 04/04/2016: cease production into the tanks, secure the load-line valves with locks, secure the enclosures with locks. Repair and secure the wellhead enclosure. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection. |
