0388172847 2016-02-22 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system and tank battery with one 210-barrel, steel production tank, and one 100-barrel, steel brine tank.The battery is shared with the Billy 13A (#2-1256). The wellhead is located in an area of active residential development. The wellhead was not enclosed. The battery is located in a separate board-on-board enclosure. Process safety and air emission control devices (low-pressure relief valve, spark arrestor, and carbon filter) were present. I walked the areas around the wellhead and battery and did not observe any signs of impact relating to the on-going production activity . Access to the well and battery was afforded by separate short, dedicated, unpaved access road which was in good condition. I observed the following deficiencies: the wellhead and battery enclosures, and load-line valves were not secured with locks. The wellhead enclosure was partially missing. Both of the storage tanks were severely corroded with perforation holes in each which have compromised the integrity of these tanks. Production into the tanks must be stopped immediately. The tanks must be removed and replaced. The owner must remedy the following by 04/04/2016: cease production into the tanks, secure the load-line valves with locks, secure the enclosures with locks. The wellhead must be in a secure enclosure. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

0980828348 2017-09-20 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: The load lines were noted to be unlocked and unsecure, the wellhead was not fenced or secure, and the 210-barrel production tank was noted to be severely corroded and required replacement. At the time of this inspection, the indicated deficiencies remained. In addition, the ID signage remained unchanged and had not been updated to reflect the change in ownership that had occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.
1047717821 2017-05-05 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves, perforated production and brine tanks, and missing, broken and unlocked fence enclosures. At the time of this inspection, I observed the following: The wellhead was comprised of a plunger-lift system with associated tank battery (shared with the #2-1256 well). There was no ID signage or enclosure at the wellhead. The load-line valves were not secured with locks. The production and brine tanks with corrosion holes remain in the battery system. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
1065638463 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead is located at the terminus of a residential street which is part of a newer subdivision. The battery is located in a partially wooded area behind residential homes along Girdled Road, west of the wellhead. The well is comprised of a mechanical plunger-lift system which is not enclosed at the time of my inspection. Fittings and connections at the wellhead are secure. The area around the wellhead is undergoing development for additional housing and significant earthwork is on-going at the time of this inspection. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure. Load-outs are locked. The containment dike is intact and dry. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable.
1384797789 2013-11-18 UrbanProduction Wells No Violations Unit is comprised of a mechanical plunger lift unit and aboveground storage tank (AST) battery. The wellhead and battery are enclosed in separate board-on-board fences. The wellhead fence has been partially torn down or damaged by vehicle impact. The battery consists of 1 100-barrel production tanks, 1 100-barrel brine tank, and separator unit. The ID placard lists an incorrect API# for this well. The secondary containment drain in the battery is not capped. The unit is producing and no other problems observed. I notified John D Oil about the ID, drain, and fence issues.
