0549004260 | 2012-07-17 | Final Restoration | No Violations | My inspection of the site found no change since last inspection. The operator needs to submit a waiver for the lease road, before final restoration can be passed. I contacted the operator and reminded him of the needed waiver. Public is using the lease road to dump trash on the property. |

0739058288 | 2017-07-18 | Final Restoration | No Violations | I inspected this well location as an evaluation for Final Restoration. The well was plugged and abandoned in 8/1991 At the time of this inspection, the area where the former well had been located has been restored with well-established grass and other vegetation. Portions of the area are in the process of being redeveloped for other use. No further activity is needed. Final Restoration is passed. |

1206372821 | 2008-03-24 | Preliminary Restoration | No Violations | Site is next to Demilta Sand and Gravel. Oil and gas well site is blocking drainage south end of permit, positive drainage will need to be obtained before reclamation is approved. Site is extremely soft. Area has received 6-8 inches of wet snow over the weekend. Drainage that was leaving the site was running clear. Pits have been closed. One frac tank on site. Tank battery parts are on site. Permit sign is up. |

1227115574 | 2008-11-19 | Production Wells | No Violations | Lease road is stable. Lease road gate needs installed. Tank battery is under construction. One 100 BBL and one 210 BBL tank in battery. Mechanical separator has not been installed yet. Well is a rabbit well. 50psi on the annulus space of the well. Positive drainage needs to be established on site, water is ponding north and south of the tank battery. Tank battery and well head needs fenced immediately. Called Tim Reilly from John D oil, left message requested that he call me regarding issues. Well is adjacent to IM-1187. Operator has 3 weeks remaining to complete reclamation. |
1408878509 | 2012-06-26 | Final Restoration | No Violations | My inspection today found the following. The well site has a history of illegal dumping of trash and construction debris before drilling. This problem has been known to me for the past 10 years. The location is adjacent to DeMilta Sand and Gravel (IM-1187). I have been the mining inspector for IM-1187 for the past 10 years. The mining operator has had to stockpile material to prevent trespassing on adjacent property. The lease road to the well site remains and is open. There has been dumping of construction debris along the lease road and around the location of the well. The lease road must be removed or a waiver from the land owner received. Establishing vegetation on location would be difficult. There was no vegetation on location before drilling. Topsoil and subsoil had been stripped before drilling by the land owner and the remaining soil is a mix of sand and fill material. This was the condition of the soil before drilling. Sedimentation is contained on location. |
1808265294 | 2016-12-17 | Final Restoration | No Violations | I inspected this former well location as part of assessment for Final Restoration. At the time of this inspection I observed the following: the well had formerly been located behind a commercial building, with access around the building via a short, access road. The area where the well had formerly been located consists of wild vegetation and open, unvegetated areas with no evidence of the well or other production equipment. There was no production equipment remaining. Final Restoration is passed. |
