0400906276 2017-09-13 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery in separate enclosures, both located on a parcel occupied by a church. The enclosure gates were locked and secure. The load-line valves were secure. Tank lids were closed and secure. The secondary containment was intact and dry. I walked the areas surrounding the well and battery and found no signs of impacts or problems.

0903497580 2012-03-30 UrbanProduction Wells Violations Noted FOLLOW UP TO NOV # -1836951035 My inspection of this well site found that the equipment spacing at the tank battery is not correct. The separator is approximately 3 from the production tank. The rules state a minimum of 10 should be maintained between these two pieces of equipment and the well owner was instructed to switch the brine tank and the production tank to meet spacing requirements. The soil of the tank battery floor is stained with crude oil, which must be removed and disposed of properly. There is a seep in the north west corner of the tank battery, where the sales line comes out of it. There is approximately one quart of free oil below the seep. There is water accumulating at the seep that has bacterial iron in it, causing red discoloring. I tested this water for chlorides and they were below drinking water standards. Oil soaked debris remains below the tank battery from a previous release from the tank battery. The well owner was instructed to clean up this debris and dispose of it properly. The valve at the south end of the load line has separated but the load line does have a second valve at the tank. The well owner has a thread protector screwed onto the south end of the load line but has been instructed to replace the valve. This valve needs to be located inside of the dike. The well owner was notified, by phone, during inspection of the needed clean up and repairs.
0971332273 2010-12-11 Production Wells No Violations The clean up was progressing. Two siphon dams were constructed and additional sorbent boom was deployed. Two vacuum trucks and a water truck were in use to recover the oil.

1352299148 2012-11-06 UrbanProduction Wells Violations Noted I inspected this unit as a follow-up to the inspection and subsequent issuance of a Compliance Notice issued on 03/30/2012. At that time, the owner was advised to make repairs to address non-compliance with the following items: The mechanical separator is 24 inches from the existing production tank. The rules mandate a separation distance of 10 feet. The tank battery dike is failing at the northwest corner where a seep is actively draining. There are several trash-bags filled with soil, located outside the tank battery fence that are now decaying and split open. The load-line valves located outside the dike and are not secured. These conditions which were extant on 03/30/2012 have not been addressed as of 11/06/2012. In addition, I noted that the ID sign for the unit is not readable. On 11/07/2012, I contacted the owners representative Tim Levengood of Industry Standard Supplies. He indicated that they were not aware that this issue had been established as a Compliance Notice, and had reportedly attempted to contact someone within the ODNR Oil and Gas Division to get clarification. We agreed to meet at the site on 11/14/2012 to review the compliance issues.
1352909881 2012-11-14 Preliminary Restoration No Violations Site meeting with Tim Levengood of Industry Standard Supplies regarding the need to address the items outlined in the current Compliance Notice. Tim agreed to address the following items: move the mechanical separator to the east side of the interior of the tank battery to comply with the spacing requirements of 1501:9-9-05(A)(5), repair and reinforce the secondary containment dike in the northwest corner, move the load-out valves to the interior of the battery, remove residual refuse in trash bags, fix the Unit ID, regrade the area on the north side of the dike where runoff is washing out the berm. He indicated that the repairs would be made within the next two weeks. I asked him to contact me when the issues had been adressed.
1371731696 2013-06-19 Production Wells No Violations The ID at the wellhead is still missing-so I called Tim Levengood and he indicated that it would be taken care of.
1428440000 2009-10-22 Preliminary Restoration No Violations Preliminary restoration was completed.

1608555594 2019-01-07 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was equipped with a plunger lift assembly and in production, at the time of this inspection. The well was fenced and locked with legible identification. The tank battery was comprised of one 100 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The diked tank battery was fenced with a locked door and legible identification. Lease access to the well and tank battery were stable.

1774379580 2013-10-09 UrbanProduction Wells No Violations The producing unit consists of a wellhead and aboveground storage tank (AST) battery in separate board-on-board enclosures. The wellhead is a plunger lift unit and is producing. The battery is comprised of 1 140-barrel and 1 50-barrel steel tank and separator. Secondary containment, spacing, and signage is good. Process safety equipment (spark arrestor, carbon filter, and low-pressure relief valve) are present. Gates are locked. ID placard has been replaced. No problems noted. Impact relating to the release that occurred on 12/10/2010 has been effectively remedied.
