At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a shared-use asphalt-paved road was in good condition. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance deficiency; the load line valves were not secured with locks as required, the ID signage had not been updated to reflect the recent change in ownership. I contacted the well tender (John Magnussen) and advised him about the deficiency. He indicated that the items would be fixed ASAP.

The well is accessed via a short, unpaved road that connects to the existing asphalt-paved driveway. The battery is immediately adjacent to the asphalt drive. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a board-on-board enclosure. Fittings and connections at the wellhead were secure. The battery is comprised of two 100-barrel steel production tanks and separator in separate board-on-board enclosures. Enclosure gates were locked and secure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked.

At the time of this inspection, the load-lines were not locked as required and the identification has not been updated to reflect current ownership. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate. I did not observe any leaks, spills, or other signs of contamination. The gates are locked.
1423841950 2015-02-13 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well is located in a commercial/light industrial area on the north side of Crile Rd. The well is comprised of a wellhead with mechanical plunger lift located 200 feet from the tank battery. The battery, mechanical separator, and battery are in separate board-on-board enclosures. The enclosure for the separator is unlocked and I found the gate to be left open. Load outs at the battery are bull-plugged. Connections and fittings are secure and free from signs of leakage or problems. At the time of this inspection, there are 10 to 15 inches of snowfall, which hinders a thorough inspection of surficial features (e.g. access road, secondary containment interior. I notified Tim Reilly (John D. Oil & Gas) about the need for a lock on the gate. I will follow-up in 30 days.

Fittings and connections at the well and battery were secure and free from signs of leakage. The secondary containment was intact. I noted the following deficiencies: the load-lines were not locked as required. The ID signage had not been updated to reflect the change of ownership that occurred over 12 month prior to this inspection. The owner must place locks at the load-line valves and place compliant unit identification at the well within 30 days or be subject to additional enforcement.
