0375417872 2013-07-15 UrbanProduction Wells Violations Noted The producing unit consists of a wellhead and concoil unit and an aboveground storage tank (AST) battery with one steel 140-barrel production tank, one steel 50-barrel brine tank, mechanical separator.. The wellhead and battery are in separate board-on-board enclosures which are locked. The unit ID is present at the tank battery but is not visible from outside. There is no ID at the wellhead which is located remote, and out of the line of sight from the tank battery. The battery secondary containment is good. The tank battery lacking the safety equipment as required by 1501:9-9-05E. I contacted Steve Franks of Petro Evaluation Services and advised him that the ID signage needs displayed at both the battery and wellhead, and safety equipment needs deployed at the battery before the start of the Lake County Fair.

0949370800 2017-04-06 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection I observed the following: A wellhead with plunger-lift system in board-on-board enclosure with associated tank battery in separate board-on-board enclosure. Fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. The areas surrounding the well and battery are comprised of equine facilities and grounds of the Lake County Fairgrounds. I walked the areas surrounding the wellhead and battery and found no signs of impact or degradation to the surrounding surficial areas.

1243611621 2019-01-14 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was equipped with a Concoil plunger lift assembly and in production, at the time of this inspection. The well was fenced with a locked man door. The tank battery was comprised of one 100 BBL tank, one 50 BBL tank, and a mechanical separator. Valves on the separator were open. The chart on the sales line showed production. The production tank was equipped with correct lids and hatches. A charcoal filter has been plumbed into the vent stack of the 100 BBL tank and topped with a spark arrestor. The tank battery was diked and fenced with a locked man door. Legible identification was present at the well and tank battery. Access to the well was stable.
