The secondary containment dike in the battery is completely missing in sections. The corrections should be made within 90 days.

1455633625 2016-02-16 UrbanProduction Wells Violations Noted I inspected this well location as par6t of routine inspection activity. At the time of this inspection I observed the following elements: A wellhead with mechanical plunger lift system, a mechanical separator, and a tank battery system in separate board-on-board enclosures. Access to the wellhead is via the shared, asphalt-paved driveway. Fittings and connections at the well and battery were secure and free from evidence of problems or leakage. The wellhead enclosure gate was unlocked due to a broken hasp. The load-line valves are not secured with locks as required. The tanks are not fitted with spark arrestors, vent stack low-pressure relief valves, or carbon-filter. The owner must remedy the following by 03/25/2016: secure the wellhead gate with a lock, secure the load-line valves with locks, fit spark arrestors, low-pressure relief valves and carbon filter at the tank battery. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
1492612254 2017-04-19 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves and missing process safety equipment (spark-arrestor, low-prsssure relief valve, carbon filter). At the time of this inspection, I observed the following: The load-line valves were not secured with locks. The tank battery was In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
