In addition, the ID signage remained unchanged and had not been updated to reflect the change in ownership that had occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.
0883329592 2017-05-05 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves, perforated production and brine tanks, and broken and unlocked fence enclosures. At the time of this inspection, I observed the following: The wellhead was comprised of a pump jack system with associated tank battery. The load-line valves were not secured with locks. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.

The wellhead includes a mechanical plunger lift located in a 6-foot chian-link enclosure. The mechanical separator is located whithin a third, separate enclosure. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. No violations noted.
1421336672 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead is located behind the referenced house on Girdled Rd. The wellhead and battery are located on a level grassy area behind, and slightly above the house. The well is comprised of a pump jack system in a chain-link enclosure Fittings and connections at the wellhead are secure. The battery is comprised of one 100-barrel low boy steel production tank and one 100-barrel low boy brine tank and separator unit in separate chain-link enclosures. Fittings are secure. Load-outs are locked. The containment dike is intact and dry. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable.

1982293486 2016-02-09 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. The well and associated battery are located behind the single-family house at this location. The well and battery are accessed via a short, unpaved road that enters from Girdled Road. At the time of this inspection, I observed the following: a wellhead with gas-fired pump jack system in a chain-link enclosure. Fittings and connections at the wellhead were secure. The battery is comprised of two 100-barrel steel production tanks, and separator in a separate chain-link enclosure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked and enclosure gate at the battery was unlocked. The owner must remedy the following by 04/08/2016: secure the load-line valves and entry gate with locks. Place ID signage at the entry gate. Fit the new production tank with spark arrestor, low-pressure relief valve, and carbon filter as required. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
