0970116925 2019-01-07 UrbanProduction Wells No Violations I conducted an inspection on this well. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. There were two domestic lines plumbed off the 4.5 inch casing with regulators, drip bottles and meters. Both domestic line valves were open. The well was fenced, locked and had legible identification. The tank battery was comprised of one 140 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The tank pick-up lines were locked. Lids and hatches on the 100 BBL tank were correct. There was also a Dominion East Ohio master meter present in the tank battery. The diked tank battery was fenced and locked with proper, legible identification. The lease access was stable.
1297890744 2014-05-13 UrbanPreliminary Restoration No Violations I inspected the location of the existing producing unit for preliminary restoration. The producing unit is comprised of a wellhead with valve tree and mechanical plunger lift system The wellhead is enclosed in a chain-link fence. There is signage at the wellhead. There are two domestic meter tie-ins located at the wellhead. The associated aboveground storage tank (AST) battery is comprised of 1 100-barrel steel production tank, 1 50-barrel steel brine tank, and horizontal separator unit. There is also a Dominion metering run locate within the battery, which is enclosed in a chain-link fence. Secondary containment is present at the battery and is in good condition. There is ID signage at the battery. The load-outs are bull-plugged. The access road to the battery and well are shared with the farm operation. The wellhead is located beyond the battery in an adjacent pasture. The former drilling pits have been filled and graded and vegatation around the wellhead and battery are well established. Drainage ap;pears to be dominated by infiltration and no signs of gully formation or washouts were observed. I did not observe any signs of spills, leakage, or problems. The requirements for Preliminary Restoration are met.
1442906459 2017-11-29 Production Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on a larger, farm parcel. At the time of this inspection, I found the following: a wellhead plunger-lift unit and associated battery in separate chain-link enclosures. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The shared-use access road was in good condition. The well was in active production. The chart recorder was active. Production reports and insurance were current.
1584241337 2012-04-30 UrbanProduction Wells No Violations On 4/30/2012, I completed an urban production well inspection status check for DNR Inspector Tom Hill. My inspection found the well head was a plunger lift unit (rabbit) surrounded by a chain link fence with the proper identification posted. The tank battery contained one 100 barrel oil storage tank, one 30 barrel brine tank, and a separator surrounded by a chain link fence with the proper identification posted. I noticed no visible spills or leaks. The lease road was in good condition and there are no violations at this time.
