Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments

-560832751 2017-12-13 0:00 UP No Violations
0560832751 2017-12-13 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: The load lines were not locked and secure. At the time of this inspection, I observed the following: the load-line valves remained unlocked. In addition, the ID signage had not been updated to reflect the change of ownership that occurred in 2016. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
1121578476 2016-03-02 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system located in a low, flat-lying area adjacent to Big Creek. A tank battery comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank, and a mechanical separator is located in the upland area west of the creek. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were present. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. The 8.625-inch casing was exposed but not gauged as required by specific permit condition. The owner must remedy the following by 04/03/2016: secure the load-line valves with locks. Expose and property gauge the 8.625-inch surface casing annulus as required by permit condition. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
1606007924 2011-12-13 UrbanPreliminary Restoration No Violations The tank battery is equipped with one 210 barrel production tank, one 100 barrel brine tank and a heat treat separator. The equipment spacing is correct (50 between production tank and separator). Spacing is also correct between well and tank battery. This site falls under urban requirements. It is secluded and on the owner/operators property. The estate is gated and there is a stream crossing that isolates the well. The well is in a fenced field and the tank battery is in a wooded location. Urban requirements are waived at this time. If there is an ownership change Urban equipment will need to be installed.
