0026207957 2019-03-26 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 1456329608, regarding the removal of the 100 BBL tank, due to corrosion and open holes in the tank. At the time of this inspection, the 100 BBL tank has not been removed and still needs removed from the site. The tank is not able to be repaired, due to large portions of the sides of the tank that are missing. Remove the 100 BBL tank from the site. The well was not in production, at the time of this inspection.
0144286144 2017-12-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the 210-barrel and the 100-barrel stock tanks were found to be severely corroded with numerous holes rendering them not usable. At the time of this inspection, I observed the following: the noted tanks were still in place, but not being produced into. In addition, the ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. I also noted that the portion of the access road imediately in front of the well and battery has become severely washed out and in poor condition. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.
0544386511 2018-07-13 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: improper identification, and severe rusting/corrosion of the production tank and brine tank. At the time of this inspection, I observed the following: the corroded tanks remain in place, but were not being actively produced into. The unit identification remained unchanged and reflected the previous owner. The well was shut-in with no current, active production. I had previously contacted the company on several occasions since the issuance of the compliance notice regarding the deficiencies. The company had made pledges to remedy the deficiencies.
0765123826 2017-09-22 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: The 100-barrel brine tank was noted to be severely corroded with large holes and required replacement. At the time of this inspection, the indicated deficiencies remained. In addition, the ID signage remained unchanged and had not been updated to reflect the change in ownership that had occurred in 2016. The 210-barrrel production was also noted to be severely corroded with several perforating holes. Repeated calls to the owner have not produced any demonstrable action or results.
0788644254 2018-02-13 Production Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: improper identification, and severe rusting/corrosion of the production tank and brine tank. At the time of this inspection, I observed the following: the corroded tanks remain in place, but were not being actively produced into. 1315420835 2011-09-06 Production Wells Violations Noted Dike inadequate to prevent oil and brine from escaping. The packing on the pumpjack is leaking, and a puddle of water appearing to be brine sits at the base of the packing. Contacted Jim Williams of Great Plains Exploration regarding the problem. Jim stated he would remedy the problems as soon as possible. No other violations at this time.
1370015790 2013-05-31 UrbanProduction Wells No Violations The producing unit consists of the following elements: a wellhead with pump jack, and a tank battery system. There are no enclosures. The tank battery is comprised of one 210-barrel and one 100-barrel steel tank, and mechanical separator. Secondary containment is present at the tank battery. Unit ID is present. Unit spacing is in accordance with the regulations. I did not observe any problems or violations.
1376970004 2010-09-02 Production Wells No Violations No violations were found at the time of inspection.

1456329608 2016-02-24 Production Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with pump jack system and tank battery with one 210-barrel production tank, and one 100-barrel, steel brine storage tank. I walked the areas around the wellhead and battery and did not observe any signs of impact relating to the on-going production activity. Access to the well and battery was afforded by a shared, unpaved access road which was in good condition. The 100-barrel brine tank has failed integrity due to penetrating corrosion. The owner must remedy the following by 04/08/2016: remove the corroded tank from service and from the location. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
