0031603353 2013-07-16 UrbanPreliminary Restoration No Violations The producing unit consists of a wellhead and pumpjack unit and an aboveground storage tank (AST) battery with one steel 140-barrel production tank, one steel 20-barrel brine tank, mechanical separator, and metering run. The unit ID is present and meets requirements. The load-outs are bull-plugged. The battery secondary containment is good. The wellhead and battery are in separate chain-link enclosures. The tank battery is shared with the #2 Unit (3408521178). Safety equipment is present in accordance with 1501:9-9-05E. The wellhead requires an ID placard. I did not observe any evidence of leakage or spills. I contacted Bruce Close with Alliance and advised him of the need for an ID placard.

1280944268 2017-03-17 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection I observed the following: a wellhead with plunger lift system and associated battery located on property owned by the Andrews Osborne school, along the east side of the Chagrin River. There is a dedicated, gravel-paved access road that enters from Ridge Road, and provides access to both the #2-1177 and #2-1178 wells. The access road was well drained and in good condition. Access gates were locked and secure. The load-line valves were closed but were not locked. I also observed that the carbon filter canister was severely coroded and the granular carbon material was spilling out of the canister. Tank lids were closed and secure. I walked the areas around the wellhead and battery and found the natural conditions intact with no signs of impact or problems. I contacted well tender Matt Postelwaite and advised about the need for locks and replacement of the carbon vapor filter. He indicated that the repairs would be made within the week.
1307982505 2011-06-13 UrbanProduction Wells No Violations Fencing in place, gate locked. Tank battery contains 2-100 bbl production tanks and mechanical seperator. Dike is good, spacing is good. Lighting arrestor and charcoal filter in place. Vent hatches in place and closed. Lease road is stable. Wellhead is a pumpjack in good repair, wellhead is fenced and locked. No violations at this time.
1312824989 2011-08-08 UrbanProduction Wells No Violations Pipeline rupture noted while conducting preliminary restoration inspection. Crude oil and brine bubbling at the surface approximately 50 yds south of pumping unit, between tank battery and pumping unit. Tested and confirmed presence of chlorides using Quantab test strips. Contacted Bruce Close of Alliance Petroleum regarding the matter. Pumper onsite at 2:45, shut well in and applied peatsorb to affected area. Will return on 8/9/11 to excavate and repair pipeline. Returned on 8/9/11 to inspect clean up. Affected area has been excavated and pipeline repaired. No evidence of oil or brine at the site. Contacted pumper regarding status of cleanup, he stated he was unable to work on ruptured piece of pipe due to groundwater infiltrating into exacavated area. He will resume repairs when a pump truck is available to removed water from the hole.
