The battery, separator, and wellhead are in separate board-on-board enclosures. The battery is comprised of 1 210-barrel steel production tank, one 100-barrel steel brine tank, and separator unit. There is adequate secondary containment. There is no safety equipment as required by 1501:9-9-05E. Locks and signage are present. No signs of leaks, spills or problems. Preliminary restoration is pending the installation of safety equipment.
0549237256 2016-02-17 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of my inspection I observed the following elements: a wellhead with mechanical plunger lift system, mechanical separator, and tank battery comprised of one 210-barrel, and one 100-barrel, steel storage tanks. The battery and wellhead were enclosed in separate board-on-board enclosures which were unlocked. Fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems with the following exception: the tank load lines were not secured with locks as required. The owner must remedy the following by 03/25/2016: secure the load-line valves and enclosure gates with locks. 1246446018 2017-09-01 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: the load-lines were no secured with locks. In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.
1357229771 2012-12-17 UrbanProduction Wells No Violations The unit is comprised of a two-tank battery (1 210-barrel production and 1 100-barrel brine) and a wellhead with mechanical plunger lift, and mechanical separator, each in separate wood board-on-board enclosures. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate although the dike drain is open. I did not observe any leaks, spills, or other signs of contamination. The gates are locked. I called Tim Reilly with Great Plains and advised him about the open dike drain. He advised that the item would be addressed by the pumper.
1512569104 2017-12-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves and enclosure gates were not locked. At the time of this inspection, I observed the following: there were no locks at the load-line valves and wellhead enclosure gates. In addition, the ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. The tank battery gate was missing board sections and the wellhead enclosure was partially broken down. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.

To date, the owner has not remedied the deficiencies and not provided the division with a schedule for when they will return to compliance.
