At the time of this inspection, I observed the following: The load-line valves were not secured with locks. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.

The mechanical separator is housed within a separate fenced enclosure. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05. Unit identification is present and legible at the tank battery. The tank battery dike is acceptable. I did not observe any leaks, spills, or other signs of contamination. The gates at the tank battery, well, and mechanical separator have locks. Due to the residential location, the tanks should have spark arrestors, low-pressure relief valves, and charcoal canister filter in accordance with HB 278. I called Jim Willimans and he said he would have this remedied within the next week.

1456755211 2016-02-29 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following elements: one wellhead with mechanical plunger lift system in a board-on-board enclosure, one mechanical separator in a board-on-board enclosure, a tank battery comprised of one 210-barrel steel tank and one 100-barrrel steel tank with associated fittings and tubular connections in a board-on-board enclosure. I observed the system fittings and connections to be secure and free from leakage. The tank lids were closed and secure. Spark arrestor, low-pressure relief valve, and carbon filter were present. Secondary containment was intact. I walked the areas surrounding the wellhead and battery and did not observe any evidence of impact or erosional deficits. Access to the well is afforded by the shared-use parking and drive area utilized by Willoughby City. I observed the following safety deficiency: the wellhead enclosure gate was not locked and secure, the load-lines valve was not locked as required. The owner must remedy the following by 03/31/2016: secure the enclosure gates and load-lines with locks. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.
