0143665515 2016-04-14 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following: one wellhead with pump jack system located in an open area on a farmstead. Fittings and connections at the wellhead were secure with the following exception: oil and other produced fluids have leaked from the stuffing box fitting and impacted an approximate 50 square foot area around the wellhead. The battery is comprised of 210-barrel production tanks, and one 50-barrel brine tank, with separator with associated appurtenances. Load lines were plugged and secure. Secondary containment was intact. Signage was present. Production reports were current. The owner must take the following action before 05/20/2016: remove impacted soil, grade and re-seed the area, repair/replace stuffing box seals and packing. 0504539708 2013-06-18 UrbanProduction Wells Violations Noted Previously I had inspected this producing unit and observed oily staining on the production tank and in the gravel material around the tank. I had indicated to the owner/operator the need to clean the oil. Upon this visit, I discovered that another spill or overfill of the production tank had occurred. Approximately 50% of the granular fill inside the secondary containment is saturated with free-phase oil. There are three areas radiating from the north side of the containment dike where oil has migrated through the dike and killed the grass. These areas extend up to 30 feet to the north. Absorbent peat moss material has been placed inside the dike and along the oily migration pathways to the north. I contacted Pierre Delafransisco with Allied Oil Field Serv. and advised him to shut-in the unit and begin remediation of the oil spill. I indicated that remediation should included removal of the oil-impacted soil outside the dike, and removal and replacement of the oil and oil-impacted granular fill inside the dike. Also the areas of dead grass would need to be re-seeded. He indicated that work would commence by mid-week next week.

The area on the north side of the tank battery dike which had been impacted with oil has been cleaned and seeded. The impacted soil pile has been removed. No further action is needed with regards to the former oil spill.
1141090144 2017-09-19 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: there had been an area of stressed vegetation/brine scald near the wellhead . At the time of this inspection, the area is regaining vegetation and so the noted deficiency had been mitigated. I walked the areas surrounding the well and battery and found no signs of problems or impacts.

The pile of impacted soil remains on location. The impacted area has not been re-seeded as required. Allied reports that these items will be remedied as soon as possible.
1372699182 2013-07-01 UrbanProduction Wells No Violations I visited the site pursuant to my earlier discussions with Allied Oil Field Services. Based on my observations today, I advised that there is still a significant amount of free oil in the dike, within the gravel. The dike is failing in several places allowing oil to migrate down-gradient. I recommended that an interceptor trench be dug on the down-gradient side of the dike to prevent further migration. More of the oily gravel needs to be removed and replaced.
1373389435 2013-07-09 UrbanProduction Wells No Violations The previously documented and observed oil leaking through the dike is continuing. Allied placed two oil sorbent booms but oil is leaking through the dike. I contacted Allied and advised them that immediate action would be required.

A small cut-off trench has been dug and a sorbent boom has been placed in it. Allied also added a sealable dike drain constructed of 1.5-inch PVC. Ive discussed with Allied the need for further mitigation and restoration consisting of reseed of the areas down-gradient of the dike with dead grass.
2086243103 2013-08-28 UrbanProduction Wells No Violations The oily impact inside the diked area and down-gradient of the tank battery has been mitigated. There is a small amount of free-phase oil in the battery. There is a newly-placed dike drain which was left open and unattended. The impacted areas outside the dike still need to be re-seeded, and there is a stockpile of impacted soil still waiting for off-site disposal.
