At the time of this inspection, I observed the following: a wellhead with pump jack system and associated battery located on an otherwise undeveloped parcel. Fittings and connections at the wellhead and battery were free from leakage. The secondary containment was intact and free from signs of spills, leaks, or other problems. The gravel-paved access road was in good condition. I observed the following deficiencies: The load-line valves were not locked or secure. The ID signage has not been updated to reflect the change in ownership that occurred in 2016. The owner must resolve these deficiencies by 10/13/17 or be subject to additional enforcement action. Please contact inspector Stephen Tompkins at 330-907-4647with any questions about this inspection.

I observed the wellhead and tank battery areas to be in generally good condition. The tank spacing is in accordance with 1509:9-9-05. The tank battery dike is in good condition. I did not observe any leaks, spills, or other signs of contamination. No violations noted.

The well is located north of Riverside Commons Drive on an elevated portion of made-land created as part of the construction of St. Rt. 2. The wellhead and battery are encompassed on the north and west sides by the St. Rt. 2 and the interchange with Lost Nation Road. My inspection noted the following: The well is comprised of a wellhead pump jack and tank battery with 1 210-barrel production tank and 1 100-barrel brine tank. At the time of my inspection, the area is covered with up to 14 inches of snow and, as such, elements of the system are obscured with snow cover. Connections and fittings are secure and free from signs of leakage or problems. Evaluation of battery interior, lease road condition, and other surficial features is not possible due to snow cover.

At the time of this inspection, I observed the following: The short unpaved access road from Riverside Commons Drive was in good condition. The production elements consisted of the following: wellhead with pump jack system tank battery with one 210-barrel steel tank, one 100-barrel steel tank, mechanical separator unit. The well was in production. The 210-barrel tank was exhibiting some exterior corrosion but was intact. Secondary containment was intact. Fittings and connections were secure and free from signs of leakage.
